摘要:
目的：观察三黄四物散外敷治疗0级糖尿病足的临床疗效及其对血清炎性因子的影响。方法：将0级糖尿病足患者80例随机分为对照组及治疗组各40例，对照组予西医常规治疗，治疗组在对照组基础上加用三黄四物散外敷。4周为1个疗程，连续治疗1个疗程后观察治疗前后两组患者临床疗效、中医证候、下肢感觉神经传导速度及血清炎性因子水平变化情况。结果：治疗组临床疗效及中医证候有效率分别为92.5%(37/40)、87.5%(35/40)，分别高于对照组的70.0%(28/40)、72.5%(30/40)，差异有统计学意义(P＜0.05)；治疗后两组患者下肢感觉神经传导速度快于治疗前(P＜0.01)，且治疗组快于对照组(P＜0.05)；肿瘤坏死因子α(tumor necrosis factor α，TNF-α)、白细胞介素1(interleukin-1，IL-1)及白细胞介素6(interleukin-6，IL-6)在治疗第7天两组均上升，对照组上升更明显(P＜0.05)；治疗第14天、21天、28天，两组TNF-α、IL-1及IL-6水平均下降，治疗组降低较对照组显著(P＜0.05)。两组均未见明显不良反应。结论：三黄四物散封包外敷可有效改善0级糖尿病足患者临床症状及下肢感觉神经传导速度，降低血清炎性因子水平，且安全有效。
Abstract:
Objective: To observe clinical effects of Sanhuang Siwu powder for external application in the treatment for diabetic foot of grage 0 and its influnce on serum inflammatory factors. Methods: Eighty patients were randomized into the control group and the treatment group, 40 cases each group, the control group accepted conventional therapy of Western medicine, and the treatment group Sanhuang Siwu powder for external application on the foundation of conventional therapy of Western medicine. Four weeks were one course, to observe clinical symptoms, TCM syndromes, sensory nerve conduction velocity of lower extremity and the levels of serum inflammatory factors before and after treating between both groups after they were treated for one course consecutively. Results: The effective rates of clinical effects and TCM syndromes in the treatment group were 92.5%(37/40) and 87.5%(35/40) respectively, higher than 70.0%(28/40) and 72.5%(30/40) of the control group, and the difference had statistical meaning(P＜0.05); after treating, sensory nerve conduction velocity of lower extermity in both groups was faster than that before treating (P＜0.01), and the treatment group was faster than the control group (P＜0.05); the levels of TNF-α, IL-1 and IL-6 lifted after treating for seven days in both groups, the rise of the control group was more notable (P＜0.05); the levels of TNF-α, IL-1 and IL-6 lowered in both groups after treating for 14 days, 21 days and 28 days, the decrease of the treatment group was more notable (P＜0.05). No obvious side effects happened in both groups. Conclusion: Sanhuang Siwu powder for external application in the treatment for diabetic foot of grage 0 could effectively improve clinical symptoms, lower the levels of serum inflammatory factors, and it is safe and effective.
