摘要:
目的：观察中西医联合治疗儿童急性感染性鼻－鼻窦炎的减毒增效作用。方法：将急性鼻－鼻窦炎患儿106例随机分为观察组51例，对照组55例。对照组服用阿莫西林克拉维酸钾7∶1制剂或克拉霉素，分早晚2次口服；观察组在对照组治疗的基础上加用自拟方治疗，每日1剂；两组均连服2周。观察患儿治疗1周、2周时病情变化情况，并评价疗效。结果：治疗1周后总有效率观察组为58.8%(30/51)，对照组为56.4%(31/55)，两组比较差异无统计学意义（P＞0.05）；治疗2周后总有效率观察组为82.4%(42/51)，对照组为76.4%(42/55)，两组总有效率比较差异无统计学意义（P＞0.05）；观察组治疗后血清白细胞介素4(interleukin-4，IL-4)水平比治疗前下调，差异有统计学意义（P＜0.05），而血清肿瘤坏死因子α(tumor necrosis factor α，TNF-α)较治疗前增高，差异有统计学意义（P＜0.05）。两组不良反应发生率比较，观察组低于对照组，差异有统计学意义（P＜0.05）。结论：解表清里法联合西药治疗儿童急性感染性鼻-鼻窦炎疗效优于单纯西药治疗，且不良反应发生率低，其疗效可能与免疫调节有关。
Abstract:
Objective: To observe the attenuated and synergetic effects of integrative medicine in the treatment for acute pediatric rhinitis-nasosinusitis. Methods: All 106 children were randomized into 51 cases of the observation group and 55 cases of the control group. The control group were given amoxicillin clavulanate potassium preparation (7∶1) or clarithromycin, twice each day, in the morning and evening; the treatment group were treated with selfmade prescriptions based on Western medicine, one dose each day; both groups took the drug for two weeks consecutively. To observe the disease changes one week and two weeks after treating, and assess clinical effects. Results: Total effective rate of the observation group one week after treating was 58.8%(30/51), higher than 56.4%(31/55) of the control group, and the difference had no statistical meaning in total effective rates between both groups(P＞0.05); two weeks after treating, total effective rate of the observation group was 82.4%(42/51), higher than 76.4%(42/55) of the control group, and the difference had no statistical meaning(P＞0.05), the levels of IL-4 reduced in the observation group after treating compared with before treating, and the difference showed statistical meaning(P＜0.05), while the levels of TNF-α lifted notably, and the difference showed statistical meaning (P＜0.05). The observation group was lower than the control group in the incidence of adverse reaction, and the difference showed statistical meaning (P＜0.05). Conclusion: Exterior-relieving interior-clearing therapeutic method combined with Western medicine is effective in the treatment for acute pediatric rhinitis-nasosinusitis with low incidence of adverse reaction, and its effects might be related to immunoregulation.
