摘要:
目的观察血癥汤加减联合西药治疗晚期胃癌的临床疗效及其对血清癌胚抗原（carcino embryonic antigen，CEA）、糖类抗原199（carbohydrate antigen 199，CA199）的影响。 方法选择晚期胃癌患者82例，按随机数表法分为观察组和对照组，每组41例。对照组给予卡培他滨联合奥沙利铂治疗，观察组在对照组治疗基础上同时服用血癥汤。观察两组临床疗效、证候评分、肿瘤标志物、免疫功能及凝血因子水平、毒副反应情况。 结果疾病控制率、客观缓解率观察组分别为92.68%（38/41）、58.54%（24/41），对照组分别为75.61%（31/41）、36.58%（15/41），观察组均高于对照组（P＜0.05）；观察组证候评分均低于对照组（P＜0.05）；观察组CEA、CA199、纤维蛋白原、D-二聚体水平低于对照组（P＜0.05）；观察组CD4+、CD4+/CD8+水平高于对照组，CD8+水平低于对照组（P＜0.05）；观察组Ⅲ～Ⅳ级毒副反应发生率［9.76%（4/41）］低于对照组［26.83%（11/41）］（P＜0.05）。 结论血癥汤加减联合西药治疗晚期胃癌有利于控制疾病进展，抗肿瘤疗效确切，可显著降低血清CEA、CA199水平，调节患者免疫功能和凝血状态，减轻化疗毒性反应。
Abstract:
ObjectiveTo study clinical effects of modified Xuezhen decoction and Western medicine in treating advanced gastric cancer and its influence on CEA and CA199. MethodsAll 82 patients were chosen, and divided into the observation group and the control group according to random number table method, 41 cases in each group. The control group was treated with capecitabine combined with oxaliplatin, and the observation group with modified Xuezhen decoction simultaneously based on western medicine. To observe clinical effects, TCM syndromes scales, tumor markers, immune function, the levels of coagulation factors and side effects in both groups. ResultsDisease control rate and objective remission rate of the observation group were 92.68% (38/41) and 58.54% (24/41), higher than 75.61% (31/41) and 36.58% (15/41) of the control group; TCM syndrome integrals of the observation group were lower than these of the control group (P<0.05); the levels of CEA, CA199, fibrinogen and D-dimer of the observation group were lower than these of the control group (P<0.05); the levels of CD4+ and CD4+/CD3+ of the observation group were higher than these of the control group, the levels of CD3+ lower than these of the control group (P<0.05); the incidence of side effects of grade Ⅲ to Ⅳ of the observation group was 9.76% (4/41), lower than 26.83% (11/41) of the control group (P<0.05). ConclusionModified Xuezhen decoction and Western medi-cine is good for disease control in treating advanced gastric cancer, it could obtain definite effects in resisting tumor, lower the levels of CEA and CA199, regulate immune function and coagulation state, and relieve the toxicity of chemotherapy.
