摘要:
目的探讨冠心病伴发抑郁患者中医证候特征。 方法计算机检索中国知网、万方数据库、维普数据库从建库至2020年12月31日关于冠心病伴发抑郁流行病学调查、中医药辨治的临床观察或随机对照试验等文献，提取目标文献中关于冠心病伴发抑郁的中医证类及其症状、体征信息，运用Microsoft Excel软件建立中医证候特征数据库，对中医证类、证候要素、症状、体征等信息进行频数统计。 结果纳入目标文献46篇，共计3446例冠心病伴发抑郁患者，提取出20种中医证类，8个病性证素、5个病位证素、38个频率＞3%的主要症状与体征。 结论3446名冠心病伴发抑郁患者主要5种中医证类为气滞血瘀证、痰瘀互结证、肝郁气滞证、心血瘀阻证、气虚血瘀证；病性证素主要为血瘀、气滞、痰浊、气虚、火；病位证素主要为肝、心、脾、胆、肾；主要症状为胸痛、胸闷、失眠多梦、心悸、胸胁胀满或妇女乳房胀痛、气短、乏力、口干、善太息等，病因病机为因情志不遂、饮食不节、年老体虚致“阳虚血瘀”，阻遏心脉，神失濡养，最终发展成为冠心病伴发抑郁疾病。
Abstract:
ObjectiveTo explore the characteristics of TCM syndromes in patients suffering CHD and depression(CHDD）. MethodsClinical observation or randomized controlled trials（RCTs） on epidemiological investigation and TCM syndrome differentiation and treatment of CHDD were searched in CNKI, Wanfang and VIP from data establishment to December 31, 2020 using computer, after extracting TCM syndrome, symptoms and body signs of CHDD in the targeted literature, Microsoft Excel software was used to establish database of TCM syndrome characteristics, to conduct frequency statistics of TCM syndrome, syndrome elements, symptoms, body signs and others. ResultsA total of 3446 patients suffering CHDD were included in 46 target literatures, 20 TCM syndromes were extracted, including eight disease syndrome elements, five disease location syndrome elements and 38 main symptoms and signs with a frequency of more than 3%. ConclusionFive TCM syndromes of 3446 patients are Qi stagnation and blood stasis pattern, phlegm and blood stasis pattern, liver depression and Qi stagnation pattern, heart-blood stasis pattern, Qi deficiency and blood stasis pattern, disease syndrome elements are blood stasis, Qi depression, phlegm turbidity, Qi deficiency and fire; disease location syndrome elements contain liver, heart, spleen, gallbladder and kidney; the main symptoms are chest pain, chest tightness, insomnia and dreaminess, palpitation, distension of hypochondrium or breast tenderness, panting, dry mouth, frequent sighing and others, the cause and the pathogenesis are Yang deficiency and blood stasis, obstructing heart vessels and spirit losing nurturing induced by emotional upset, improper diet, aging and weakness, consequently, it develops into CHDD.
