摘要:
目的研究内热针结合关节松动术治疗冻结肩对患者中医证候、肩关节活动范围及日常生活活动能力的影响。 方法选取肩周炎患者60例为研究对象，根据不同的治疗方式，划分为观察组和对照组，每组患者30例。对照组采用关节松动术，观察组采用内热针+关节松动术结合方式治疗。比较两组肩关节活动指标、生活活动能力情况、疼痛畏寒情况，分析日常生活活动能力与肩关节各指标的相关性和影响情况。 结果时点效应、交互效应、组间效应下，前屈、外展、指耳距、指椎距、生活活动能力、畏寒程度均因不同组别不同时点的变化而变化，且观察组比对照组明显（P＜0.05）。疼痛评分于时点效应、交互效应均发生明显变化（P＜0.05），但是组别效应下，观察组与对照组比较差异不明显（P＞0.05）。生活活动能力与肩关节各指标相关性分析显示呈线性正相关关系，应用线性Logistics回归得出外展、指耳距可作为独立影响因素，影响患者日常生活活动能力。 结论内热针结合关节松动术治疗冻结肩患者，可缓解肩关节疼痛，改善肩关节活动功能，提高日常生活活动能力和生活质量。
Abstract:
ObjectiveTo study the effects of internal heat needle and joint loosening on TCM syndrome, range of motion (ROM) of the shoulder joint and activity of daily living (ADL) in patients with frozen shoulder. MethodsSixty patients were chosen as the subjects investigated, and randomly allocated to the observation group and the control group according to different therapeutic modes with 30 cases in each group. The control group adopted joint loosening, and the observation group was treated with internal heat needle and joint loosening. To compare the indexes of shoulder joint motion, ADL, pain and chills between both groups, to analyze the connections between ADL and the indexes of shoulder joint, as well as the influencing conditions. ResultsTime-point effects, interaction effect, between-group effect, forward flexion, abduction, finger ear distance, finger vertebral space, ADL and chill degree changed due to different time points in different groups, and the observation group was more noticeable than the control group (P<0.05). VAS showed remarkable changes with time-point effects and interaction effect (P<0.05), while under the between-group effect, the difference was insignificant between the observation group and the control group (P>0.05). The correlation analysis between ADL and different indexes of shoulder joint showed a linear positive correlation relationship, linear Logistics regression presented that abduction and finger ear distance can be used as the independent influencing factors, affecting the patients' ability to perform ADL. ConclusionInternal heat needle and joint loosening in the treatment of frozen shoulder could alleviate the pain in shoulder joint, improve the function and activity of shoulder joint, raise ADL activity and quality of life.
