摘要:
目的观察化瘀解毒汤对瘀毒内阻型脓毒症患者相关炎症指标及肠道菌群的影响。 方法将瘀毒内阻型脓毒症患者120例，随机分为对照组和观察组，每组60例。对照组予氢化可的松治疗，观察组在对照组基础上予化瘀解毒汤内服治疗，两组均连续治疗7天。比较两组急性生理学及慢性健康状况评分（acute physiological and chronic health scores，APACHEⅡ）、血清肿瘤坏死因子α（tumor necrosis factor α，TNF-α）、白细胞介素6（interleukin-6，IL-6）表达水平，并比较患者肠道菌群改变情况、临床疗效和不良反应发生情况。 结果治疗后，两组患者APACHEⅡ评分及血清IL-６与TNF-α表达水平均降低，且观察组降低程度大于对照组（P＜0.05）；乳酸杆菌、双歧杆菌数量均明显增加（P＜0.05），而肠杆菌、肠球菌数量明显减少（P＜0.05），且观察组上述菌群数量增加或降低程度大于对照组（P＜0.05）；总有效率观察组［95.00%（57/60）］高于对照组［80.00%（48/60）］（P＜0.05）；不良反应发生率观察组为8.33%（5/60），对照组为10.00%（6/60），差异无统计学意义（P＞0.05）。 结论在西医常规治疗的基础上以化瘀解毒汤治疗瘀毒内阻型脓毒症患者，能够有效控制病情进展，减轻炎症反应，改善肠道菌群失调，提高疗效，且安全性好。
Abstract:
ObjectiveTo observe the influence of stagnation-resolving detoxifying decoction (Huayu Jiedu Tang) on the related inflammatory markers and intestinal flora in blood stasis and toxin obstruction type of sepsis. MethodsAll 120 patients were randomized into the control group and the observation group with 60 cases in each group. The control group was treated with hydrocortisone, and the observation group took the decoction on the foundation of the therapy the control group accepted, both groups were treated for seven consecutive days. To measure APACHEⅡ, the levels of TNF-α and IL-6 in the two groups, and to compare the changes of intestinal flora, clinical effects and the incidences of adverse reactions. ResultsAfter the treatment, the scales of APACHEⅡ, the levels of TNF-α and IL-6 were reduced in the two groups, the decrease of the observation group was greater than that of the control group (P<0.05); bacillus acidi lactici and bacillus bifidus count was obviously added (P<0.05); while the number of enterobacteria and enterococcus was remarkably reduced (P<0.05), and the increase or decrease in the above flora in the observation group was greater than that in the control group (P<0.05); total effective rate of the observation group was [95.00%(57/60)], higher than [80.00%(48/60)] of the control group (P<0.05); the incidence of adverse reaction of the observation group was 8.33% (5/60), lower than 10.00% (6/60) of the control group, and the difference had no statistical meaning (P>0.05). ConclusionOn the foundation of conventional therapy of Western medicine, the decoction could effectively control disease progression, relieve inflammatory response, improve the imbalance of intestinal flora, increase clinical effects with good safety in the treatment of sepsis of blood stasis and toxin obstruction pattern.
