摘要:
目的分析透骨散足浴对膝关节置换术（total knee arthroplasty，TKA）后下肢深静脉血栓形成（deep venous thrombosis，DVT）的预防作用。 方法选取行初次TKA治疗的膝骨关节炎（knee osteoarthritis，KOA）患者92例，采用随机数字表法分为对照组和观察组，每组46例。对照组于术后第1天给予依诺肝素钠注射液治疗，观察组在对照组基础上于术后第1天加用透骨散足浴，两组均连续治疗2周。比较两组患者DVT发生率、下肢静脉血流速度及血浆D-二聚体含量。 结果DVT总发生率观察组［6.52%（3/46）］低于对照组［23.91％（11/46）］（P＜0.05）；术后2周，两组患者股深静脉与腘静脉血流速度明显加快，且观察组加快更明显（P＜0.05）；术后1周、2周，两组患者血浆D-二聚体含量明显降低，且观察组降低更明显（P＜0.05）。 结论透骨散足浴干预TKA术后KOA患者可减少下肢DVT发生率，改善血液高凝状态以及下肢血液循环。
Abstract:
ObjectiveTo analyze preventive effects of Tougu powder foot bath on deep venous thrombosis (DVT) of the lower extremities after TKA. MethodsA total of 92 KOA patients undergoing TKA therapy initially were chosen, and divided into the control group and the observation group according to random number table method with 46 cases in each. Enoxaparin sodium injection was administered to the control group on the first day after the operation, and the observation group received Tougu powder foot bath on the first day after the operation based on the therapy of the control group, and both groups were treated for two consecutive weeks. To compare the incidences of DVT, venous blood flow velocity in the lower extremities and the contents of plasma D-dimer between the two groups. ResultsTotal incidence of DVT of the observation group was [6.52%(3/46)], lower than [23.91%(11/46)] of the control group (P<0.05); two weeks after the surgery, blood flow velocity in the deep femoral vein and popliteal vein were increased in the two groups obviously, and the increase of the observation group was more evident (P<0.05); one week and two weeks after the surgery, the contents of plasma D-dimer were lowered evidently in the two groups, and the decrease of the observation group was more obvious (P<0.05). Conclusion Tougu powder foot bath used in intervention for KOA patients after TKA could reduce the incidence of DVT of the legs, improve hypercoagulable state of blood and blood circulation of the lower limbs.
