摘要:
目的探讨国医大师班秀文治疗崩漏的组方用药规律。 方法收集班秀文教授1970—1993年间的临证医案手稿，筛选出其中治疗崩漏的医案93例，运用SPSS 18.0以及Microsoft Excel 2019软件对医案中的所有药物进行频数、性味、归经和药效的分析，并对高频药物进行聚类分析和关联规则分析，总结班秀文教授治疗崩漏的临床经验。 结果共收集处方247首，涉及药物170味；频次≥30次的药物共27味，主要有白芍、益母草、甘草、当归、白术、党参、山药、茯苓、黄芪、覆盆子等；常用的药物组合包括“当归-川芎”“白芍-地黄”等；药性以温、平为主；药味甘、苦、辛多见；药物归肝、脾、肺、肾经最多；药效以补虚药为主。 结论班秀文教授治疗崩漏的用药特色以滋阴泻热，疏肝柔肝，补肾为要，扶脾补虚，擅用四物汤、两地汤、二至丸和五子衍宗丸等方剂。
Abstract:
ObjectiveTo explore the prescription compatibility and medication patterns of national TCM master Ban Xiuwen in treating uterine bleeding. MethodsWe collected the clinical medical records of Professor Ban Xiuwen from 1970 to 1993, screened out 93 cases of uterine bleeding, SPSS 18.0 and Microsoft Excel 2019 were used to analyze the frequency, nature and flavor, meridian tropism, and therapeutic effects of all herbs used in these medical records. Cluster analysis and association rule analysis were performed on the high-frequency herbs, in order to summarize the clinical experience of Professor Ban Xiuwen in treating uterine bleeding. ResultsA total of 247 prescriptions comprising 170 distinct herbs were collected. Twenty-seven herbs exhibited a frequency of 30 or higher, with the most frequently prescribed being Baishao (Paeoniae Radix Alba), Yimucao (Leonuri Herba), Gancao (Glycyrrhizae Radix et Rhizoma), Danggui (Angelicae Sinensis Radix), Baizhu (Atractylodis Macrocephalae), Dangshen (Codonopsis Radix), Chinese yam (Dioscoreae Rhizoma), Fuling (Poria), Huangqi (Astragali Radix), and Fupenzi (Rubi Fructus). The most common herb pairs were "Danggui-Chuanxiong" and "Baishao-Dihuang". With respect to herb properties, warm and neutral natures were predominant, while sweet, bitter, and pungent flavors were the most frequently recorded. The herbs were primarily attributed to the liver, spleen, lung, and kidney meridians. Tonifying herbs constituted the largest therapeutic category. ConclusionProfessor Ban Xiuwen’s medication characteristics in treating uterine bleeding are mainly focused on nourishing Yin and draining heat, soothing and softening the liver, and tonifying the kidney, as well as strengthening the spleen and replenishing deficiency. He was skilled in using classical formulas such as Siwu Tang (Four-Substance Decoction), Liangdi Tang (Two-Rehmannia Decoction), Erzhi Wan (Two-Ultimate Pill), and Wuzi Yanzong Wan (Five-Seed Progeny Pill).
