摘要:
目的：观察 470 例胸腰椎骨折患者行保守治疗的临床疗效。方法：对 470 例胸腰椎骨折患者实施闭合复位，中药治疗，功能锻炼，支具固定等保守治疗，分别于治疗前、治疗后 2 周进行 VAS 评分和 Cobb 角测量，并对合并脊髓 / 神经损伤的病例于治疗前、治疗后 2 周进行 ASIA 评分，记录合并症和住院时间，对相关结果进行统计学分析。结果：470 例患者中 3 例完全性脊髓损伤的胸腰椎骨折患者治疗后 2 周较治疗前 ASIA评分好转 1 级，9 例部分性脊髓损伤的胸腰椎骨折患者治疗后 2 周较治疗前 ASIA 评分好转 2 级以上。治疗前、治疗后 2 周患者 VAS 评分分别为(7.60±1.80）分、（2.30±0.80)分，差异具有统计学意义(P＜0.05)，Cobb 角分别为(24.80±6.50）度、（5.20±2.90)度，差异具有统计学意义(P＜0.05)。常见合并症有腹胀 95 例，便秘64 例。住院时间 5-38 天，平均 14.8 天。结论：只要适应证选择得当，中西医结合保守治疗胸腰椎骨折具有住院时间短、并发症少、疗效显著等优点。
Abstract:
Objective: To study clinical efficacy of conservative treatment for 470 patients suffering from thoracolumbar fractures. Method: All 470 patients received conservative treatment such as closed reduction, TCM therapy, functional exercise, orthosis fixation, VAS and Cobb angle measurement were performed before treating and in two weeks after treating, ASIA scale was presented to those who complicated with spinal cord/neural injury before treating and in two weeks after treating, the complications and hospitalization time were recorded, the data was analyzed statistically. Result: Among 470 patients, ASIA of three patients with complete spinal cord injury in two weeks after treating was improved one grade than before treating, ASIA of nine patients with injury of part of spinal cord was improved more than two grades. VAS scores of patients before treating and in two weeks after treating were (7.60±1.80) and (2.30±0.80) respectively, the differences showed statistical meaning (P＜0.05), Cobb angles were respectively the degrees of(24.80±6.50) and (5.20±2.90), the differences had statistical meaning (P＜0.05). The common complications were 95 cases with abdominal distention and 64 cases with constipation. Hospitalization time was 5 to 38 days and average length of hospital stay was 14.8 days. Conclusion: Integrative treatment presents the advantages of short hospitalization time, less complications, remarkable effects for the appropriate indications of thoracolumbar fracture.
