摘要:
目的观察自拟蠲痹通络汤治疗早期类风湿性关节炎（rheumatoid arthritis，RA）湿热痹阻证的临床疗效及对脂肪因子（内酯素、脂联蛋白、抵抗素）的影响。 方法将RA患者72例按随机数字表法分为观察组与对照组各36例。对照组予甲氨蝶呤片和柳氮磺吡啶肠溶片治疗，观察组在此基础上予自拟蠲痹通络汤治疗。治疗4周后观察两组患者治疗前后主要症状（关节疼痛、肿胀及屈伸不利）评分，检测红细胞沉降率（erythrocyte sedimentation rate，ESR）、C-反应蛋白（C-reactive protein，CRP）、类风湿因子（rheumatoid factors，RF）及血清内酯素、脂联蛋白、抵抗素等指标水平，并评价临床疗效。 结果总有效率观察组为91.7%（33/36），高于对照组的75.0%（27/36），差异有统计学意义（P＜0.05）；治疗后两组患者关节疼痛、肿胀及屈伸不利等主要症状评分，ESR、CRP、RF水平及血清内酯素、脂联蛋白、抵抗素等脂肪因子水平较治疗前下降（P＜0.05），观察组下降更明显（P＜0.05）。 结论甲氨蝶呤片和柳氮磺吡啶肠溶片联合自拟蠲痹通络汤能缓解早期RA湿热痹阻证患者主要临床症状，改善实验室理化指标，其机制可能与降低脂肪因子水平，调节炎症免疫反应有关。
Abstract:
ObjectiveTo observe clinical effects of selfmade Juanbi Tongluo decoction in treating early RA of dampness-heat obstruction pattern and its impacts on adipokines (visfatin, adiponectin and resistin). MethodsAll 72 RA patients were allocated to the observation group and the control group according to random number table method. The control group were treated by methotrexate tablets and sulfasalazine enteric-coated tablets, and the observation group by selfmade Juanbi Tongluo decoction on the foundation of Western medicine. After four weeks of treatment, to observe the scores of main symptoms (joint pain, swelling and motor impairment) of both groups before and after treating, to detect the levels of ESR, CRP, RF, visfatin, adiponectin and resistin, and assess clinical effects. ResultsTotal effective rate of the observation group was 91.7%（33/36）, higher than 75.0%（27/36） of the control group, and the difference showed statistical meaning (P<0.05); after treating, the scores of main symptoms including joint pain, swelling and motor impairment, the levels of ESR, CRP and RF, visfatin, adiponectin and resistin reduced in both groups (P<0.05), the decrease of the observation group was more notable (P<0.05). ConclusionMethotrexate tablets and sulfasalazine enteric-coated tablets combined with selfmade Juanbi Tongluo decoction could relieve main clinical symptoms of early RA patients of dampness-heat obstruction pattern, improve lab physical and chemical indexes, and its mechanism might be related to lowering the levels of adipokines and regulating inflammatory immune response.
