摘要:
目的系统评价补肾活血类中药对少弱精子症模型生殖激素水平的干预作用。 方法检索中国知网、重庆维普中文科技期刊数据库、万方数据知识服务平台、中国生物医学文献数据库、PubMed、Embase和Cochrane Library,筛选补肾活血类中药干预少弱精子症模型的实验研究,制作资料提取表并实施资料提取,采用SYRCLE动物实验偏倚风险评估工具对纳入文献的研究方法学偏倚风险进行评估,并用RevMan5.3软件对结局指标进行Meta分析。 结果共纳入10个研究,研究对象为SD大鼠、KM小鼠和BALB/c小鼠,均为随机对照实验。Meta分析结果显示,与模型组比较,补肾活血类中药可在一定程度上降低卵泡刺激素(follicle stimulating hormone,FSH)(SMD=0.14,95%CI[-0.13,0.41],P<0.00001)、黄体生成素(luteinizing hormone,LH)(SMD=-0.32,95%CI[-0.57,-0.08],P<0.00001)含量,提高睾酮(testosterone,T)(SMD=0.85,95%CI[0.61,1.08],P<0.00001)、雌二醇(estradiol,E2)(SMD=0.63,95%CI[0.07,1.20],P=0.003)、催乳素(prolactin,PRL)(SMD=0.36,95%CI[-0.27,0.99],P=0.006)含量。亚组分析结果显示,相较于模型组:补肾活血类中药低剂量组0.05~0.7 g/(kg·d)、中剂量组1~1.7 g/(kg·d)、高剂量组2~6 g/(kg·d)均可提高T含量,且差异均有统计学意义(P<0.05);补肾活血类中药高、中剂量组均可降低FSH、LH含量,且差异均有统计学意义(P<0.05);补肾活血类中药高剂量组可降低E2、PRL含量,差异均有统计学意义(P<0.05)。 结论补肾活血类中药能够有效改善生殖激素水平,且存在一定的量效关系。但是受纳入文献数量与质量要素的限制,以上结论尚待相关高质量研究的验证。
Abstract:
ObjectiveTo systematically assess the intervention effects of kidney-tonifying blood-activating herbs on reproductive hormone level in oligoasthenospermia models. MethodsBy searching from CNKI, VIP, Wanfang, CBM, PubMed, Embase and Cochrane Library, experimental studies on kidney-tonifying blood-activating herbs in the intervention of oligoasthenospermia models were screened to make data extraction table and implement data extraction, SYRCLE animal experiment bias risk assessment tool was adopted to assess methodological bias risk of the included literature, software RevMan 5.3 was used to perform Meta analysis of the outcome indexes. ResultsA total of ten studies were included, the subjects were SD rats, KM mice and BALB/c mice, all of them were randomized controlled experiments. Meta analysis results displayed that, compared with the model group, kidney-tonifying blood-activating herbs could reduce the contents of FSH (SMD=0.14, 95%CI [-0.13, 0.41], P<0.00001), LH (SMD=-0.32, 95%CI [-0.57, -0.08], P<0.00001), and lift the contents of T (SMD=0.85, 95%CI [0.61, 1.08], P<0.00001), E2(SMD=0.63, 95%CI [0.07, 1.20], P=0.003) and PRL (SMD=0.36, 95%CI -0.27, 0.99], P=0.006). Subgroup analysis in the study showed that, compared with the model group: low [0.05 to 0.7 g/(kg·d)], moderate [1 to 1.7 g/(kg·d)] and high [2 to 6 g/(kg·d)] dosages groups of kidney-tonifying blood-activating herbs could raise the contents of T, and the difference showed statistical meaning (P<0.05); high and moderate dosage groups of kidney-tonifying blood-activating herbs could reduce the contents of FSH and LH, and the difference was statistically significant (P<0.05); high dosage group of kidney-tonifying blood-activating herbs could cut down the contents of E2 and PRL, and the difference was statistically significant (P<0.05). ConclusionKidney-tonifying blood-activating herbs could effectively regulate reproductive hormone level, and there is a certain dose effect relationship. However, due to the limitation of the quantity and quality of the included literature, the above conclusions need to be verified by relevant high-quality studies.
