摘要:
目的采用Meta分析方法评价身痛逐瘀汤治疗经皮内窥镜下腰椎间盘切除术（percutaneous endoscopic lumbar discectomy，PELD）后患者残余症状的有效性与安全性。 方法计算机检索2021年9月1日前发表在中国知网、万方数据知识服务平台、维普数据库、中国生物医学文献服务系统、PubMed、Embase、Cochrane Library、Web of Science数据库中所有关于身痛逐瘀汤治疗PELD术后患者残余症状的临床随机对照研究。Meta分析采用Review Manager 5.3软件，并依据Cochrane系统评价纳入文献的质量。 结果共纳入8篇文献，涉及588例患者，其中观察组（身痛逐瘀汤组）292例，对照组（常规药物组）296例。观察组患者视觉模拟疼痛评分（visual analogue scale，VAS）（MD=-0.65，95%CI［-0.77，-0.53］，P＜0.000 0）、Oswestry功能障碍指数（oswestry disability index，ODI）（MD=-3.61，95%CI［-4.07，-3.16］，P＜0.000 01）、日本骨科协会评估治疗分数（Japanese orthopaedic association，JOA）（MD=2.25，95%CI［1.28，3.22］，P＜0.000 01）均优于对照组，差异有统计学意义（P＜0.05）。 结论身痛逐瘀汤在治疗PELD术后患者残余症状方面具有明显优势，可以缓解患者术后残余腰腿部疼痛、麻木及无力等症状，提高患者生活质量，且具有较高的安全性。但本研究结论仍需大样本、高质量文献验证其真实性与可靠性。
Abstract:
ObjectiveTo assess the effectiveness and the safety of Shentong Zhuyu decoction in the treatment of residual symptoms in patients undergoing PELD by using Meta analysis. MethodsClinical randomized controlled trials (RCTs) on Shentong Zhuyu decoction in the treatment of residual symptoms in patients after PELD published before September 1st, 2021 were searched from CNKI, WANFANG DATA, VIP, SinoMed, PubMed, Embase, Cochrane library and Web of Science by using computer, Meta analysis was performed by adopting Review Manager 5.3 software, the quality of the included literature was assessed according to Cochrane system. ResultsEight papers were included, involving 588 cases, among them, there are 292 cases in the observation group (Shentong Zhuyu decoction group), and 296 cases in the control group (conventional medicine group). VAS (MD=-0.65, 95%CI[-0.77, -0.53], P<0.000 0), ODI (MD=-3.61, 95%CI[-4.07,-3.16], P<0.000 01) and JOA (MD=2.25,95%CI[1.28, 3.22], P<0.000 01) of the observation group were superior to these of the control group, and the difference had statistical meaning (P<0.05). ConclusionShentong Zhuyu decoction shows its obvious edge in treating residual symptoms in patients undergoing PELD, it could relieve residual pain, numbness and weakness in waist and legs after the operation, raise quality of life with higher safety. While large samples and high-quality literature are still needed to verify the authenticity and reliability of the study.
