摘要:
目的系统评价温针灸联合中药内服对慢性盆腔炎（chronic pelvic inflammatory disease，CPID）患者的临床疗效。 方法计算机检索PubMed、EMbase、The Cochrane Library、万方数据、中国知网（CNKI）和中文期刊服务平台（VIP）等数据库，检索查询关于温针灸联合中药内服治疗CPID的随机对照试验（randomized controlled trial，RCT）和半随机对照试验，检索时限均从各数据库建库至2024年5月。由2位研究人员独立进行文献筛选和数据提取，并对纳入研究进行质量评价，评估可能存在的偏倚风险，采用Rev Man 5.3软件对纳入研究的有效率和中医症状积分进行Meta分析。 结果共纳入14项研究，发表时间为2011—2023年，共计1370例患者；Meta分析结果显示：观察组有效率高于对照组（RR=1.23，95%CI：1.17～1.28，P＜0.000 01），观察组中医证候积分改善优于对照组（SMD=-3.03，95%CI：-4.42～-1.64，P＜0.0001），差异均有统计学意义；14篇文献中，仅有1篇报道了不良反应，其余研究均未提及。 结论温针灸联合中药内服治疗慢性盆腔炎有一定优势，但本系统评价纳入的研究质量较低，样本量较小，尚需高质量的RCT进行进一步验证。
Abstract:
ObjectiveTo systematically assess clinical effects of warming needling moxibustion combined with traditional Chinese medicine for oral administration in the treatment of chronic pelvic inflammatory disease (CPID). MethodsRandomized controlled trials (RCT) and semi-RCT on warming needle moxibustion and oral administration of TCM in the treatment of CPID were retrieved from PubMed, EMbase, The Cochrane Library, Wanfang database, CNKI and VIP, the retrieval limit ranged between the time of database establishment and May, 2024. Two researchers independently performed literature screening and data extraction, carried out quality evaluation of the included researches, and assessed the risk of bias that might existed, Rev Man 5.3 software was adopted to perform Meta analysis of the effective rate and TCM syndrome integrals of the included studies. ResultsAll 14 studies have been included, and the publication time was between 2011 and 2023, involving 1370 patients; Meta analysis results showed that the effective rate of the observation group was higher than that of the control group (RR=1.23,95%CI:1.17～1.28,P<0.000 01), the improvements of TCM syndrome integrals of the observation group were better than these of the control group (SMD=-3.03,95%CI:-4.42～-1.64,P<0.0001), and the difference had statistical meaning; among 14 papers, adverse reaction has been reported in only one paper, and has not been mentioned in the other studies. ConclusionWarming needling moxibustion combined with oral administration of herbs has certain advantages in the treatment of CPID, however, high-quality RCTs are needed for further verifica-tion because the quality of the studies is low in this systematic evaluation and the sample size is small.
