摘要:
目的：评估临床路径的实施对医疗效率、经济类、医疗质量及中医特色指标的影响。方法：以2011年实施临床路径的混合痔手术患者作为临床路径组连续观察60例，以2010年肛肠科的混合痔手术患者作为历史对照组，分析临床路径实施前后各项指标的变化。结果：等待手术天数历史对照组(1.91±0.15)天，临床路径组(1.22±0.06)天；平均住院天数历史对照组(14.65±0.66)天，临床路径组(13.2±0.61)天。2组相比差异显著(P＜0.05)。住院总费用、西药费、中成药费、中草药费、检查费2组相比差异显著(P＜0.05)。2组治愈好转率均为100.00％。中药薰洗液使用天数历史对照组为(5.15±0.78)天，临床路径组为(6.78±0.27)，2组相比差异显著(P＜0.05)；中医辨证论治准确率历史对照组为78.33%(47/60)，临床路径组为96.67%(58/60)；中医药治疗率2组均为100.00％。结论：临床路径可改善医疗效率、经济类、医疗质量及中医特色指标。
Abstract:
Objective: To evaluate the influence of implanting clinical pathway on index of medical efficacy, economics, medical quality and TCM features. Method: Sixty patients implanting clinical pathway at the year of 2011 as group of clinical pathway were observed consecutively and compared with patients undergoing mixed hemorrhoids surgery as control group to analyze changes of indexes before and after implanting clinical pathway. Result: Patients in control group waited (1.91±0.15) days and ones in clinical pathway group did (1.22±0.06) days; average hospitalization days of control group was (14.65±0.66) days, while the days of clinical pathway group was (13.2±0.61) days. Comparison of both groups showed remarkable difference (P＜0.05). Comparison between both groups showed significant difference in total cost of hospitalization, expense of western medicine, cost of patent medicine, fee of Chinese herbal medicine and test fee (P＜0.05). Improvement rates of both groups were 100.00%. Days of using TCM lotion for control group were (5.15±0.78), the days for clinical pathway group were (6.78±0.27), comparison of both groups showed remarkable difference (P＜0.05). Accuracy rate of syndrome differentiation and treatment in control group was 78.33%(47/60), the rate of clinical pathway group was 96.67%(58/60); healing rate of two group was 100.00%. Conclusion: Clinical pathway could improve indexes of medical efficacy, economics, medical quality and TCM features.
