摘要:
目的:观察清热利湿通络方联合西药治疗类风湿关节炎湿热痹阻证的疗效及对白细胞介素1（IL-1）、肿瘤坏死因子α（TNF-α）水平的影响。方法：将类风湿关节炎湿热痹阻证患者110例随机分为西医组和中西医组各55例，西医组采用美洛昔康+甲氨喋呤治疗，中西医组在此基础上加用清热利湿通络方辅助治疗，比较2组患者总有效率、治疗前后症状体征评分、健康调查简表（SF-36）评分、红细胞沉降率（ESR）、类风湿因子（RF）、IL-1、TNF-α水平及药物不良反应发生情况。结果：中西医组总有效率高于西医组（P＜0.05）；中西医组治疗后患者关节肿痛而热、关节屈伸不利、晨僵、口渴汗出及尿黄、便干评分均低于西医组（P＜0.05）；中西医组治疗后PCS评分和MCS评分均高于西医组和治疗前（P＜0.05）；中西医组治疗后ESR和RF水平均低于西医组（P＜0.05）；中西医组治疗后关节液TNF-α和IL-1水平低于西医组（P＞0.05）；2组药物不良反应发生率比较差异无统计学意义（P＜0.05）。结论：清热利湿通络方联合西药可有效减轻类风湿关节炎湿热痹阻证患者关节症状体征，提高生活质量，降低关节炎性细胞因子水平。
Abstract:
Objective: To observe clinical effects of heat-clearing damp-eliminating collateral-dredging prescription jointed with western medicine in treating rheumatoid arthritis(RA) of damp-heat arthralgia pattern and its effects on the levels of IL-1 and TNF-α. Methods: All 110 patients were separated into the group of western medicine and the group of traditional Chinese and western medicine, 55 groups each group, they accepted meloxicam+methopterin respectively and heat-clearing damp-eliminating collateral-dredging prescriptions as adjunctive therapy based on the former therapy, total effective rate, symptom and body sign scales, SF-36 scales, the levels of ESR, RF, IL-1 and TNF-α, as well as the incidence of drug adverse reaction of the patients were compared between both groups. Results: The group of traditional Chinese and western medicine was higher than the group of western medicine in total effective rate(P＜0.05); the group of traditional Chinese and western medicine was lower than the group of western medicine in the scales of swollen, painful and hot joints, limited movement, morning stiffness, thirst, sweating, yellow urine and dry stool (P＜0.05); PCS scale and MCS scale of the group of Traditional Chinese and western medicine after treating were higher than these of the group of western medicine and these before treating (P＜0.05); the levels of ESR and RF of the group of traditional Chinese and western medicine after treating were lower than these of the group of western medicine and these before treating(P＜0.05); the group of Traditional Chinese and western medicine after treating was lower than the group of western medicine and the group before treating in the levels of IL-1 and TNF-α(P＜0.05); the difference was insignficant in the comparison of the incidence of adverse reaction between both groups(P＞0.05). Conclusion: Heat-clearing damp-eliminating collateral-dredging prescription jointed with western medicine could effectively mitigate the symptoms and body signs of the patients of RA of damp-heat arthralgia pattern, improve quality of life, lower the level of arthritis cytokines.
