摘要:
目的：探讨经皮穴位电刺激联合耳穴贴压药丸对剖宫产产妇恶心呕吐干预的效果及机制。方法:将80例剖宫产产妇随机分为观察组和对照组，每组各40例，2组均进行常规剖宫产手术，观察组同时给予经皮穴位电刺激干预联合耳穴贴压药丸治疗，观察患者不同阶段恶心、呕吐、低血压、低心率发生率、肛门首次排气时间和术后6小时出血量，并且应用放射免疫法测定治疗前后五羟色胺(5-HT)水平。结果：T0时刻，平均动脉压(MAP)、心率(HR)、血样饱和度(SpO2)组间比较，差异无统计学意义(P＞0.05)，与T0相比，2组在T1时刻MAP降低、HR升高，对照组MAP在T2、T3、T4时刻均降低，HR在T2时刻升高，差异有统计学意义(P＜0.05)，MAP、HR、SpO2在同一时刻2组比较，差异无统计学意义(P＞0.05)。低血压、低心率、恶心、呕吐发生率观察组均低于对照组，差异有统计学意义(P＜0.05)。在T1、T2、T3、T4时段恶心、呕吐视觉模拟评分观察组均低于对照组，差异有统计学意义(P＜0.05)。随着时间延长，恶心、呕吐视觉模拟评分2组均下降，同组内相邻2个时段比较，差异均有统计学意义(P＜0.05)。肛门首次排气时间2组比较，差异有统计学意义(P＜0.05)。术后6小时出血量2组比较，差异无统计学意义(P＞0.05)。5-HT水平手术前后2组组内比较，差异有统计学意义(P＜0.05)；术后组间比较，差异有统计学意义(P＜0.05)。结论：经皮穴位电刺激联合耳穴贴压药丸可有效降低剖宫产产妇恶心呕吐发生率，可能是通过降低5-HT水平而发挥治疗作用的。
Abstract:
Objective: To explore the intervention effects and the mechanism of Transcutaneous Electrical Acupoint Stimulation (TEAS) and auricular plaster with pills on nausea and vomiting of puerperants who underwent cesarean section. Methods: Eighty patients were randomized into the observation group and the control group, 40 cases each group, both groups accepted conventional cesarean section, the observation group were treated by TEAS and auricular plaster with pills, the incidence of nausea, vomiting, hypotension and low heart rate, anal first exhaust time and bleeding amount in six hours after the operation of the patients were observed at different phases, the levels of 5-HT were detected before and after treating by radioimmunoassay. Results: The difference had no statistical meaning in the comparisons of MAP, HR and SpO2 at T0 time (P＞0.05), compared with the time of T0, MAP decreased, HR rose in both groups at the time of T1, MAP decreased at the times of T2, T3 and T4 in the control group, HR rose at the time of T2, and the difference had statistical meaning (P＜0.05), the difference had no statistical meaning in the comparisons of MAP, HR and SpO2 between both groups at the same time (P＞0.05). The observation group was lower than the control group in the incidences of nausea, vomiting, hypotension and low heart rate, and the difference had statistical meaning (P＜0.05). The observation group was lower than the control group in visual analogue scores of nausea and vomiting at the times of T1, T2, T3 and T4, and the difference was statistically significant (P＜0.05). As the time went by, VAS of nausea and vomiting decreased in both groups, compared with these at two adjacent times within the same group, the difference showed statistical meaning (P＜0.05). There was a significant difference in the comparisons of first exhaust time of anal between both groups (P＜0.05). The difference had no statistical meaning in the comparisons of bleeding amount in six hours after the operation (P＞0.05). The difference showed statistical meaning in the comparisons of the level of 5-HT between before and after the operation within the same group (P＜0.05); the difference showed statistical meaning in the comparisons between both groups after the operation (P＜0.05). Conclusion: TEAS and auricular plaster with pills could effectively lower the incidences of nausea and vomiting of puerperants who underwent cesarean section, which might achieve therapeutic effects by decreasing the level of 5-HT.
