摘要:
目的：观察耳穴贴压对全麻妇科腹腔镜术患者镇痛效果及胃肠功能的影响。方法：将104例患者按随机数字表法分为观察组和对照组，每组52例。观察组采用耳穴贴压法，于耳穴神门、胃、交感及皮质下用胶布粘住王不留行籽贴在耳穴处，按压3～5 min，连续2天。对照组采用安慰疗法，只在相应耳穴上粘贴外观相同的胶布，但不进行王不留行籽粘贴及按压，连续2天。于术后1 、24、48 小时，采用疼痛视觉模拟量表(VAS)评估2组患者疼痛程度。观察2组首次肛门排气时间、排便时间、肠鸣音恢复时间、术后24 小时内恶心呕吐发生率。结果：术中麻醉时间、手术时间、气腹压力及异丙酚和舒芬太尼用量2组比较，差异均无统计学意义(P＞0.05)。VAS评分随着时间的增长，2组患者均持续降低(P＜0.05)；各时间点观察组均低于同期对照组(P＜0.05)。术后首次肛门排气时间、排便时间及肠鸣音恢复时间观察组均少于对照组(P＜0.05)。恶心呕吐发生率观察组为19.23%，低于对照组48.08%(P＜0.05)。结论：耳穴贴压对于全麻妇科腹腔镜术患者可明显缓解其疼痛，改善其胃肠功能，降低恶心呕吐发生率。
Abstract:
Objective: To survey the effects of auricular pressure on analgesic effects and gastrointestinal function of the patients undergoing gynecological laparoscopy under general anesthesia. Methods: All 104 patients were allocated to the observation group and the control group according to random number table method, 52 cases each group. The observation group adopted auricular pressure, sticking the seed of cowherb to the ear points including Shenmen, Wei, Jiaogan and Pizhixia, pressing for three to five minutes, for two days consecutively. The control group accepted placebo therapy, sticking the same rubber cloth to the same ear points, but no pressing for two days consecutively. Pain degrees of the patients in both groups were assessed by VAS in one hour, 24 hours and 48 hours, initial anal exhaust time, defecation time and the recovery time of bowel sound of both groups were observed, and the incidence of nausea and vomiting within 24 hours after the operation were compared between both groups. Results: The difference had no statistical meaning in the comparisons of intraoperative anesthesia time, surgical time, pneumoperitoneum pressure and the dose of Propofol and sulfentanyl between both groups (P＞0.05). VAS scores decreased gradually as the time increased (P＜0.05); the observation group was lower than the control group at different times(P＜0.05). The observation group was less than the control group in initial anal exhaust time, defecation time and the recovery time of bowel sound (P＜0.05). The incidence of nausea and vomiting of the observation group was 19.23%, lower than 48.08% of the control group (P＜0.05). Conclusion: Auricular pressure could relieve the pain obviously, improve gastrointestinal function and decrease the incidence of nausea and vomiting of the patients undergoing gynecological laparoscopy under general anesthesia.
