摘要:
目的观察清热化瘀汤治疗子宫内膜异位症（endometriosis，EMs）的疗效及其对原激活蛋白激酶（mitogen activated protein kinase，MAPK）/细胞外信号调节激酶（extracellular signal-regulated kinase，ERK）通路的影响。 方法选择EMs患者122例，根据随机数字法分为观察组和对照组，每组61例。对照组予以米非司酮治疗，观察组在对照组治疗的基础上予以清热化瘀汤治疗。比较两组临床疗效，中医证候、痛经、月经量评分，盆腔包块体积，子宫血流动力学［搏动指数（pulsatility index，PI），阻力指数（resistance index，RI），收缩末期最大血流速度（systole，S）/舒张期对最大血流速度（diastolic phase，D）］，促卵泡成熟激素（follicle-stimulating hormone，FSH）、黄体生成激素（luteinizing hormone，LH）、雌二醇（estradiol，E2）和孕酮（progesterone，P）、MAPK，ERK及血管内皮生长因子（vascular endothelial growth factor，VEGF）水平的变化。 结果观察组总有效率为93.44%（57/61），明显高于对照组［78.69%（48/61）］（P＜0.05）。治疗后两组中医证候、痛经、月经量评分，盆腔包块体积，PI、RI、S/D、FSH、LH、E2、P、MAPK、ERK和VEGF水平均较治疗前明显降低（P＜0.05），而观察组降低幅度较对照组更为明显（P＜0.05）。 结论清热化瘀汤治疗EMs疗效显著，可缓解痛经症状，改善子宫血液循环，纠正激素紊乱，其机理与抑制MAPK/ERK通路的活性有关。
Abstract:
ObjectiveTo observe clinical effects of heat-clearing stasis-resolving decoction in the treatment of EMs and its influence on MAPK/ERK pathway. MethodsAll 122 EMs patients were selected and divided into the observation group and the control group according to random number table method,61 cases in each group.The control group was treated by mifepristone,and the observation group was given heat-clearing stasis-resolving decoction based on Western medicine.To compare clinical effects,the scores of TCM syndrome,dysmenorrhea and menstrual volume,the volume of pelvic mass, uterine hemodynamics(PI,RI,S/D),the levels of FSH,LH,E2,P,MAPK,ERK and VEGF between both groups. ResultsTotal effective rate of the observation group was 93.44%(57/61),notably superior to 78.69%(48/61) of the control group(P<0.05).After treating, the scores of TCM syndrome,dysmenorrhea and menstrual volume,the volume of pelvic mass, PI, RI, S/D, the levels of FSH, LH, E2, P, MAPK, ERK and VEGF of two groups after treating lowered notably compared with these beore treating(P<0.05),while the decrease of the observation group was more notable than that of the control group(P<0.05). ConclusionHeat-clearing stasis-resolving decoction could gain notable effects in the treatment of EMs,it could relieve the symptoms of dysmenorrhea,improve uterine blood circulation and correct hormone disturbance,and its mechanism is related to inhibiting the activity of MAPK/ERK pathway.
