摘要:
目的观察穴位按压联合阳光心态护理模式对老年高血压患者血压控制效果及心理状态的影响。 方法选取198例老年高血压患者，根据不同干预方式分为对照组（穴位按压）85例和观察组（穴位按压联合阳光心态护理模式）113例，比较两组干预前后血压控制情况、证候积分、心理症状评分［汉密顿焦虑量表（Hamilton anxiety scale，HAMA）、贝克抑郁问卷（Beck depression inventory，BDI）］和遵医行为，比较两组干预后生活质量评估量表（quality of life，QOL）评分和护理满意度。 结果干预方式和时间存在交互效应，随着时间延长，两组收缩压均有下降，观察组下降幅度更明显（P＜0.05）；不同时间点两组舒张压存在明显差异，随着时间延长，舒张压逐渐降低（P＜0.05）；干预后两组眩晕、头痛、心悸气短等证候积分均低于干预前（P＜0.05）；干预后观察组HAMA、BDI评分低于对照组（P＜0.05）；干预后两组遵医行为均有提升，观察组用药治疗、饮食指导等遵医行为高于对照组（P＜0.05）；观察组干预后QOL评分均高于对照组（P＜0.05）；观察组满意度为［92.04%（104/113）］，高于对照组［82.35%（70/85）］（P＜0.05）。 结论对老年高血压患者采用穴位按压联合阳光心态护理模式，可有效控制血压，改善心理症状，提高生活质量和患者满意度。
Abstract:
ObjectiveTo observe the effects of acupoint pressing and sunshine mindset nursing model on blood pressure control and psychological state in the elderly patients with hypertension. MethodsAll 198 patients were chosen, and divided into 85 cases in the control group (acupoint pressing) and 113 cases in the observation group (acupoint pressing and sunshine mindset nursing model) according to intervention modes, to compare blood pressure control conditions, syndrome integrals, psychological symptoms scores [HAMA and BDI], and thera-peutic compliance between both groups before and after the intervention, to compare QOL scores and nursing satisfaction between both groups after the intervention. ResultsInteraction effects existed between the interven-tion mode and time, as the time went on, systolic blood pressure in both groups was lowered, the decrease of the observation group was more notable (P<0.05); significant difference could be found in diastolic blood pressure at different times, as time went on, diastolic blood pressure was reduced gradually (P<0.05); after the intervention, the scores of the syndromes including vertigo, headache, palpitation and panting in both groups were lower than these before the intervention (P<0.05); HAMA and BDI scores of the observation group were lower than these of the control group (P<0.05); therapeutic compliance was improved in both groups after the intervention, and the observation group was higher than the control group in therapeutic compliance including medication and dietary guidance (P<0.05); the observation group was higher than the control group in QOL scores after the intervention (P<0.05); the satisfaction rate of the observation group was 92.04%, higher than 82.35% of the control group (P<0.05). ConclusionAcupoint pressing and sunshine mindset nursing model for the elderly patients with hypertension could effectively control blood pressure and improve psychological symptoms, raise quality of life and the patients' satisfaction degrees.
