摘要:
目的归纳针灸治疗全膝关节置换术（total knee arthroplasty，TKA）术后急性疼痛的选穴规律。 方法以“关节置换”为主题词，并含“针灸”或“针刺”或“艾灸”或“穴位”或“电针”等，检索中国知网数据库、维普中文期刊服务平台、万方数据库中登载的针灸治疗TKA术后急性疼痛的临床研究文献。将符合标准的文献进行整理，建立针灸处方数据库，分析其选穴特点及规律。 结果共纳入文献29篇，入选腧穴37个，累计使用频次127次。针灸治疗TKA术后急性疼痛常用的6个腧穴依次为血海（17次）、足三里（16次）、阳陵泉（13次）、阴陵泉（9次）、梁丘（9次）、三阴交（7次），腧穴归属最多的经络为足阳明胃经，其次为足太阴脾经，最常用的特定穴为五腧穴。 结论针灸治疗TKA术后急性疼痛以近部取穴为主，常取穴于血海、足三里、阴陵泉、阳陵泉、梁丘；常与足太阴脾经和足阳明胃经腧穴配伍使用，配伍方法多以表里经配伍法为主。
Abstract:
ObjectiveTo sum up the law of selecting the acupoints for acute pain after total knee arthroplasty (TKA) with acupuncture. MethodsChoosing "joint replacement" as the key words, containing "acupuncture" or "needling" or "moxa-moxibustion" or "acupoint" or "acusector", clinical research literature on treating acute pain after TKA was retrieved from CNKI, VIP and Wanfang. The literature meeting the criteria were sorted out to build the database of acupuncture prescription, and then to analyze the characteristics of acupoint selection and the law. ResultsAltogether 29 papers were included, involving 37 acupoints, with the cumulative frequency of 127 times. Six commonly-used acupoints for acupuncture in the treatment of acute pain after TKA contained Xuehai (SP10, 17 times), Zusanli (ST36, 16 times), Yanglingquan (GB34, 13 times), Yinlingquan (SP9, 9 times), Liangqiu (ST34, 9 times), and Sanyinjiao (SP6, 7 times), the meridian that the acupoints mostly belong to was stomach meridian of foot Yangming, spleen meridian of foot Taiyin, the most commonly-used specific acupoints were Five shu points. ConclusionAcupuncture therapy for acute pain after TKA basically choose the acupoint from the neighboring part, usually including Xuehai, Zusanli, Yinlingquan, Yanglingquan and Liangqiu; commonly combining with the acupoints of stomach meridian of foot Yangming, spleen meridian of foot Taiyin, adopting external and internal meridians as the compatibility method.
