摘要:
目的探讨生化汤加味方联合米索前列醇治疗稽留流产的疗效及对血清缺氧诱导因子1α（hypoxia inducible factor-1α，HIF-1α）、血管内皮生长因子（vascular endothelial growth factor，VEGF）、β-人绒毛膜促性腺素（β-Human chorionic gonadotropin，β-hCG）水平的影响。 方法将稽留流产患者90例按照随机数字表法分为观察组和对照组各45例，对照组患者予米非司酮片、米索前列醇片治疗；观察组在对照组基础上联合生化汤加味方治疗，孕囊排出后仍持续服用生化汤加味方2周。比较两组患者临床疗效、一般治疗情况及治疗前与治疗1周、2周血清HIF-1α、VEGF、β-hCG水平，观察治疗期间不良反应发生情况。 结果总有效率观察组为75.56%，高于对照组的44.44%，差异有统计学意义（P＜0.05）；观察组患者孕囊排出时间、阴道出血时间和出血量均短于/少于对照组，差异有统计学意义（P＜0.05）；两组患者治疗1周、2周血清HIF-1α、VEGF、β-hCG水平均低于治疗前，观察组低于对照组，差异有统计学意义（P＜0.05）；两组患者不良反应发生率比较差异无统计学意义（P＞0.05）。 结论生化汤加味方联合米索前列醇治疗稽留流产疗效优于米索前列醇治疗，且能调节患者血清HIF-1α、VEGF、β-hCG水平。
Abstract:
ObjectiveTo discuss curative effects of modified Shenghua formulation in combination with misoprostol in the treatment of missed abortion and its impact on the levels of serum HIF-1α, VEGF and β-hCG. MethodsAll 90 patients were allocated into the observation group and the control group according to random number table method, the control group was treated by mifepristone tablets and misoprostol tablets; and the observation group took modified Shenghua formulation on the foundation of the therapy the control group accepted, and they continued to take the decoction for two consecutive weeks till the fertilized eggs discharged. To compare clinical effects, general therapy, the levels of serum HIF-1α, VEGF and β-hCG before the treatment, after one week and two weeks of the treatment, to observe the incidence of adverse reaction during therapeutic period. ResultsTotal effective rate of the observation group was 75.56% higher 44.44% that of the control group, and the difference had statistical meaning (P<0.05); the observation group was shorter/less than the control group in the fertilized egg discharge time, duration and volume of vaginal bleeding, and the difference was statistically significant (P<0.05); after one week and two weeks of the treatment, the levels of serum HIF-1α, VEGF and β-hCG were lower than these of both groups before the treatment, the observation group was lower than the control group, the difference had statistical meaning (P<0.05); the difference had no statistical meaning in the incidence of adverse reaction (P>0.05). ConclusionModified Shenghua formulation jointed with misoprostol are better than only misoprostol in the treatment of missed abortion, and the therapy could adjust the levels of serum HIF-1α, VEGF and β-hCG.
