摘要:
目的探讨蒋健教授对郁证性心悸的诊治经验。 方法通过数据挖掘的方法，包括频数分析、关联规则分析、聚类分析，回顾性分析2017年12月至2019年12月在蒋健教授门诊被诊断为郁证性心悸的病例，从一般情况、证候、症状、用药4个方面分析。 结果102份病案中以中年女性为主，共涉及160味中药，85种症状表现，证候主要以心神失养证、心气不足证、肝气郁滞证、瘀阻心脉证、心胆气虚证、心脾两虚证为主。症状纷繁复杂，前3位是气短乏力、寐差、善悲欲哭。用药频次前3位是柴胡、酸枣仁、龙骨，主要方剂是柴胡加龙骨牡蛎汤、酸枣仁汤、龙胆泻肝汤、温胆汤、甘麦大枣汤。 结论郁证性心悸以补心安神、疏肝解郁、温胆化痰、健脾养心为主要治疗原则。
Abstract:
ObjectiveTo discuss the experience of diagnosing and treating depressive palpitation. MethodsData mining methods, including frequency analysis, association rules analysis, cluster analysis, to retrospectively analyze the medical cases diagnosed as depressive palpitation in professor Jiang Jian's outpatient between December, 2017 and December, 2019 from general condition, syndrome, symptoms and medication. ResultsMost of 102 medical cases are middle-aged woman, involving 160 herbs and 85 symptoms, and the syndromes contain cardiac spirit losing nourishment, cardiac Qi insufficiency, liver Qi stagnation, stagnation obstructing cardiac pulse, Qi deficiency of heart and gallbladder, deficiency of heart and spleen primarily, the symptoms are complex, and the top three are shortness of breath, lassitude, poor sleeping, susceptible to sorrow and would like to cry. The top three herbs in the frequency of drug use are Chaihu (Bupleuri radix), Suanzaoren and Longgu (fossil fragments), and the main prescriptions are Chaihu plus Longgu Muli tang, Suanzaoren Tang, Longdan Xiegan Tang, Wendan Tang and Ganmai Dazao Tang. ConclusionThe main therapeutic principle of depressive palpitation is to nourish the heart and calm the mind, disperse stagnated liver Qi to relieve depression, warm the gallbladder and resolve phlegm, invigorate spleen and nourish the heart.
