摘要:
目的：观察健脾消痞汤联合穴位贴敷治疗慢性阻塞性肺疾病急性加重期(AECOPD)胃肠功能障碍的临床疗效。方法：将60例患者采用隐匿数字随机法分为观察组、对照组各30例。2组均给予抗感染、平喘、化痰及支持治疗等对症治疗，观察组在对症治疗的基础上给予健脾消痞汤联合穴位贴敷，对照组在对症治疗的基础上给予吗叮啉，10 mg/次，3次/d，口服。2组均以治疗7天为1个疗程，1个疗程后比较2组胃肠功能、肠鸣音及炎症指标改善情况。结果：治疗后2组胃肠功能评分、白细胞数和C反应蛋白(CRP)均较治疗前明显降低，差异有统计学意义(P＜0.05)；肠鸣音较治疗前明显提高，差异有统计学意义(P＜0.05)；观察组治疗后胃肠功能评分、肠鸣音增加次数、白细胞数和CRP降低幅度均大于对照组(P＜0.05)。总有效率观察组为73.33%，对照组为33.33%，2组疗效比较，差异有统计学意义(P＜0.05)。结论：健脾消痞汤联合穴位贴敷对改善慢性阻塞性肺疾病急性加重期胃肠功能障碍作用明显，可减轻炎症反应，增加肠鸣音，减轻病情严重程度，提高预后。
Abstract:
Objective: To observe clinical effects of JianPi XiaoPi Tang combined with acupoint application in treating gastrointestinal dysfunction of the patients with chronic obstructive pulmonary disease at acute exacerbation (AECOPD). Methods: Sixty patients were randomized into the observation group and the control group according to hidden figures random method. Both groups accepted symptomatic treatment including anti-inflammation, relieving the asthma, eliminating the phlegm and supportive treatment. The observation group accepted JianPi XiaoPi Tang combined with acupoint application on the foundation of symptomatic treatment, and the control group took domperidone on the basis of symptomatic treatment, 10 mg each time, three times per day, oral administration. Seven days were one course of the treatment, the improvements of gastrointestinal function, intestinal gurgling sound and inflammatory indexes were compared between both groups after treating in one course of the treatment. Results: Gastrointestinal function scales, white blood cells (WBC) counting and C-reactive protein (CRP) of both groups after treating decreased compared with these before treating notably, and the difference showed statistical meaning(P＜0.05); intestinal gurgling sound improved remarkably compared with before treating, and the difference demonstrated statistical meaning(P＜0.05); the observation group was larger than the control group in gastrointestinal function scales, the increasing times of intestinal gurgling sound, WBC counting and the decrease of CRP(P＜0.05). Total effective rate of the observation group was 73.33%, higher than 33.33% of the control group, and the difference had statistical meaning in the comparison of curative effects(P＜0.05). Conclusion: JianPi XiaoPi Tang combined with acupoint application could improve gastrointestinal dysfunction of the patients with AECOPD obviously, relieve inflammations, increase intestinal gurgling sounds, relieve the disease and raise the prognosis.
