摘要:
目的：观察陇中正骨手法联合经皮椎体成形术(PVP)治疗骨质疏松性椎体压缩骨折的临床疗效。方法：纳入符合标准的患者56例，其中陇中正骨手法联合PVP治疗者28例，PVP治疗者28例；所有患者分别于术前、术后观察记录疼痛视觉模拟评分(VAS)、椎体矢状面Cobb角变化、Oswestry功能障碍指数以及骨水泥渗漏率的发生，并将上述指标进行比较。结果：2组患者治疗后疼痛及功能均具有明显改善，术前和术后VAS评分及ODI功能障碍指数组内比较差异有统计学意义(P＜0.05)；术前及术后组间比较，2组VAS评分及ODI功能障碍指数差异均无统计学意义(P＞0.05)；2组患者在术前Cobb角比较差异无统计学意义(P＞0.05)，陇中正骨手法联合PVP组Cobb角与术前比较明显改善且差异无统计学意义(P＜0.05)；PVP组与术前比较Cobb角无明显改善，差异无统计学意义(P＞0.05)，2组患者术后Cobb角比较，陇中正骨手法联合PVP组较单纯PVP组改善明显差异有统计学意义(P＜0.05)；陇中正骨手法联合PVP与单纯PVP在骨水泥的渗漏率比较，差异有统计学意义(P＜0.05)。结论：陇中正骨手法联合PVP与单纯PVP治疗均能迅速缓解OVCF患者的疼痛，能早期提高患者的日常生活能力，陇中正骨手法联合PVP能更好的矫正脊柱后凸畸形，但两者都不能避免骨水泥渗漏的发生。
Abstract:
Objective: To observe clinical effects of Longzhong bone-setting manipulation joined with percutaneous vertebroplasty (PVP) in treating osteoporotic vertebral compression fracture (OVCF). Methods: Among 56 patients who met the inclusion criteria, there were 28 cases who accepted Longzhong bone-setting manipulation plus PVP, and 28 cases who were treated by PVP; visual analogue scale (VAS), Cobb angles of vertebral body sagittal plane, Oswestry disability index(ODI) and the leakage rate of bone cement of all the patients were observed and recorded before and after the surgery respectively, and all the indexes were compared. Results: The pain and functions of the patients in both groups have been improved obviously after treating, there were significant differences in the comparisons of VAS scores and ODI within the groups before and after the operation(P＜0.05), there was no significant difference in the comparisons of VAS scores and ODI between both groups before and after surgery (P＞0.05); there was no difference in the comparisons of Cobb angle between both groups before the surgery (P＞0.05), Cobb angle in the group of Longzhong bone-setting manipulation plus PVP improved obviously compared with before the surgery, and the difference had no statistical meaning(P＜0.05), when PVP group was compared with before surgery, Cobb angle didn′t improve notably, and the difference had no statistical meaning (P＞0.05), the group of Longzhong bone-setting manipulation plus PVP improved more notably compared with the PVP group in the comparisons of Cobb angles between both groups after the surgery (P＜0.05); there was obvious difference in the comparisons of the leakage rate of bone cement between both groups(P＜0.05). Conclusion: Longzhong bone-setting manipulation plus PVP and single PVP could relieve the pain of OVCF rapidly, improve daily living ability of the patients early, Longzhong bone-setting manipulation plus PVP could correct Kyphosis deformity better, while both couldn′t avoid the incidence of bone cement leakage.
