摘要:
目的：探讨健脾润肤汤对特应性皮炎脾虚证患者血清细胞因子白细胞介素1、4、25(IL-1、IL-4、IL-25)、肿瘤坏死因子α(TNF-α)及皮疹严重程度的影响。方法:将特应性皮炎患者60例按随机数字表法分为观察组和对照组各30例，2组外用维生素E乳膏外涂患处每天2次，观察组同时口服健脾润肤汤每天2次，对照组口服湿毒清片每天3次，共治疗4周。采用AD(SCOREAD)皮损评分于治疗前后评价患者特应性皮炎皮损情况，并检测患者血清细胞因子IL-1、IL-4、IL-25、TNF-α。结果:总有效率观察组为58.6%，对照组为28.6%，2组比较差异有统计学意义（P＜0.05）。皮损AD分值观察组从治疗前的(43.46±14.60)分下降至(22.99±13.30)分，对照组从(46.59±12.20)分下降至(31.30±14.70)分。2组治疗后IL-4、IL-25较治疗前下降，观察组IL-1较治疗前上升，但差异无统计学意义(P＞0.05)，2组治疗后TNF-α比较差异有统计学意义(P＜0.05)。结论：健脾润肤汤可能通过调节血清Th1/Th2细胞因子的失衡发挥治疗特应性皮炎、减轻皮损的作用。
Abstract:
Objective: To discuss the influence of spleen-invigorating skin-moisturizing decoction on the levels of IL-1, IL-4, IL-25and TNF-αof the patients suffering atopic dermatitis (AD) of spleen deficiency pattern, as well as the severity of rash. Methods: Sixty patients were randomized into the observation group and the control group according to random number table method, 30 cases each group, both groups accepted vitamin E cream for external use on the affected urea, twice each day,the observation group took spleen-invigorating skin-moisturizing decoction orally, twice per day, and the control group ShiDuQing tablets, 3 times each day, they were treated for four weeks. The levels of IL-1, IL-4, IL-25and TNF-αof the patients were detected and the conditions of skin lesions were evaluated before and after treating by the score of AD(SCOREAD).Results: Total effective rate of the observation group was 58.6%, higher than 28.6% of the control group, and the difference showed statistical meaning (P＜0.05). SCOREAD scores of the observation group were (43.46±14.60) points before treating, decreased to (22.99±13.30) points after treating, the scores of the control group were (46.59±12.20) points, decreased to (31.30±14.70) points. The levels of IL-4 and IL-25 of both groups after treating lowered compared with before treating, the level of IL-1 of the observation group raised compared with before treating while the difference had no statistical meaning(P＞0.05); the difference had statistical meaning in the comparison of the level of TNF-α between both groups after treating (P＜0.05). Conclusion: Spleen-invigorating skin-moisturizing decoction could play the rols of treating AD and relieving skin lesion by regulating the unbalance of serum Th1/Th2 cytokine.
