摘要:
目的探讨比较抚触和穴位按摩配合蓝光照射治疗新生儿黄疸的疗效。 方法检索PubMed、EMbase、Web of science、Cochrane、CBM、中国知网、万方、维普等文献数据库。按照Cochrane handbook评价系统查找抚触护理和穴位按摩治疗新生儿黄疸的随机对照试验，进行数据提取和质量评价后，采用RevMan5.3软件进行Meta分析。 结果共纳入19篇文献，23个研究，共2064名患者。Meta分析结果显示：抚触护理联合蓝光与单独蓝光治疗黄疸有效率比较，差异有统计学意义［RR=1.16，95%CI（1.08，1.26），P=0.000 2］，患儿黄疸经过治疗后，血清总胆红素水平差异有统计学意义［MD=-29.52，95%CI（-39.56，-19.47），P＜0.000 01］，患儿治疗后黄疸消退时间比较，差异有统计学意义［MD=-1.89，95%CI（-2.50，-1.29），P＜0.000 01］；穴位按摩联合蓝光对黄疸治疗有效率差异有统计学意义［RR=1.14，95%CI（1.01，1.28），P=0.04］，能降患儿黄疸治疗后血清总胆红素水平［MD=-27.10，95%CI（-30.71-，-23.50），P＜0.000 01］，患儿治疗后黄疸消退时间差异有统计学意义［MD=-1.40，95%CI（-1.99，-0.80），P＜0.000 01］。 结论抚触护理联合蓝光治疗能增加黄疸治疗有效率，可明显降低患儿黄疸治疗后血清总胆红素水平，缩短黄疸消退时间；穴位按摩联合蓝光能提高黄疸治疗有效率，降低患儿黄疸治疗后血清总胆红素水平，缩短黄疸消退时间。
Abstract:
ObjectiveTo explore and compare clinical effects of touch, acupoint massage combined with blue light irradiation in the treatment of neonatal jaundice. MethodsBy searching PubMed, EMbase, Web of science, Cochrane, CBM, CNKI, Wanfang, VIP and other literature databases, randomized controlled trials (RCTs) on touch and acupoint massage in treating neonatal jaundice were sought according to Cochrane handbook evaluation system, after performing data extraction and quality evaluation, RevMan5.3 software was used for meta-analysis. ResultsA total of 19 articles and 23 studies involving 2064 patients were included. Meta analysis results showed that there was statistical difference in therapeutic effective rates of touch combined with blue light irradiation and only blue light irradiation in treating jaundice [RR=1.16, 95%CI(1.08，-1.26), P=0.000 2], the difference was statistically significant in the levels of serum total bilirubin after treating [MD=-29.52, 95%CI (-39.56，-19.47), P<0.000 01], the disappearing time of jaundice of the children showed statistical difference [MD=-1.89, 95%CI(-2.50，-1.29), P<0.000 01]; therapeutic effective rates of acupoint massage combined with blue light irradiation in treating blue light demonstrated statistical difference [RR=1.14, 95%CI(1.01，1.28), P=0.04], it could lower the levels of serum total bilirubin after treating [MD=-27.10, 95%CI(-30.71，-23.50), P<0.000 01], the disappearing time of jaundice after treating had statistical difference [MD=-1.40, 95%CI(-1.99，-0.80), P<0.000 01]. ConclusionTouch combined with blue light irradiation could lift therapeutic rate of the jaundice, lower the levels of serum total bilirubin after treating notably, shorten jaundice disappearing time of the children after treating; acupoint massage combined with blue light irradiation could increase therapeutic effective rate of the jaundice, decrease the levels of serum total bilirubin after treating, and shorten jaundice disappearing time of the children after treating.
