摘要:
目的探讨贫血对晚期胃癌患者预后及生活质量的影响。 方法依据晚期胃癌患者入组时基线血红蛋白值将其分为轻度贫血组、中重度贫血组与非贫血组，通过晚期胃癌患者中位生存时间观察贫血对患者预后的影响；将晚期胃癌患者根据中药干预时间，以3个月为界，分为足量暴露组（3个月以上）和非足量暴露组（不足3个月），分析中药暴露对晚期胃癌贫血患者预后的影响；采用《肿瘤常见症状及中医症状调查表》评估晚期胃癌患者症状及其生活质量。 结果贫血与晚期胃癌患者生活质量和生存预后呈负相关；相较于非贫血患者，肿瘤合并贫血患者具有预后不良、临床症状严重、生活质量差等特点。 结论中医药可能通过提高患者血红蛋白水平来改善患者预后。
Abstract:
ObjectiveTo discuss the influence of anemia on the prognosis and quality of life of patients with advanced gastric cancer. MethodsThe patients with advanced gastric cancer were divided into mild anaemia group, moderate to severe anaemia group and non-anaemia group according to the baseline hemoglobin values at enrolment, to observe the effects of anemia on the prognosis of the patients by their median survival time; according to the intervention time of TCM, the patients, based on a three-month period, were separated into full exposure group (longer than three months) and non-fully exposed group (less than three months), to analyze the impacts of TCM exposure on the prognosis of the patients; questionnaire on common symptoms of tumor and TCM symptoms was adopted to assess the influence of symptoms on the patients’ quality of life. ResultsAnemia was negatively associated with quality of life and survival prognosis in gastric cancer patients; compared with non-anemia patients, the patients with tumor and anemia showed the characteristics of poor prognosis, severe clinical symptoms and poor quality of life. ConclusionTCM could improve the prognosis possibly through raising the levels of hemoglobin.
