摘要:
目的观察低频脉冲穴位电刺激联合基于镜像神经元系统理论的动作观察疗法在缺血性卒中吞咽障碍患者中的应用效果。 方法选取102例缺血性脑卒中吞咽障碍患者，随机分为对照组与观察组，每组51例。两组患者入院后均接受常规康复训练，对照组给予基于镜像神经元系统理论的动作观察疗法，观察组在对照组基础上给予低频脉冲穴位治疗，两组均治疗1个月。比较两组患者中医症状评分、舌喉复合体活动度、洼田饮水试验分级、血清营养性指标、神经损伤指标及不良反应。 结果干预后观察组患者饮水呛咳、吞咽困难、舌强语謇、流涎等症状评分，视锥蛋白样蛋白、神经元特异性烯醇化酶、抗衰老关键蛋白抗体、血浆神经肽Y表达水平低于对照组（P＜0.05）；舌骨前移、舌骨上移、甲状软骨前移、甲状软骨上移活动度，以及血清前白蛋白、白蛋白、血红蛋白水平均高于对照组（P＜0.05）；两组患者洼田饮水试验结果相比差异有同学意义（P＜0.05），且观察组Ⅰ～Ⅲ级者占比明显高于对照组（P＜0.05）；观察组不良反应发生率［7.84%（4/51）］与对照组［1.96%（1/51）］比较，差异无统计学意义（P＞0.05）。 结论低频脉冲穴位电刺激联合基于镜像神经元系统理论的动作观察疗法有利于修复缺血性卒中吞咽障碍患者受损神经，提高舌喉复合体活动度，缓解相应临床症状，改善机体营养状态。
Abstract:
ObjectiveTo study application effects of low frequency pulse acupoint electrical stimulation + action observation therapy based on mirror neuron system theory on patients with swallowing disorder after ischemic stroke. MethodsA total of 102 patients with swallowing disorder of ischemic stroke were selected and divided into the control group and the observation group with 51 cases in each group. Both groups accepted conventional rehabilitation training after admission, the control group was given action observation therapy based on mirror neuron system theory, and the observation group was treated with low frequency pulse acupoint electrical stimulation, and both groups were treated for one month. To compare TCM symptom scores, laryngopharyngeal complex mobility, water swallowing test, serum nutritional indicators and nerve damage index and adverse reactions between both groups. ResultsAfter the intervention, the scores of the symptoms including choking, dysphagia, tongue stiffness and speech impediment, drooling, and the expressions of visinin-like protein, neuron-specific enolase, anti-aging key protein antibody and plasma neuropeptide Y of the observation group were lower than these of the control group (P<0.05); The mobility of hyoid bone anterior displacement, hyoid bone superior displacement, thyroid cartilage anterior displacement, thyroid cartilage superior displacement, the levels of serum prealbumin, albumin, and hemoglobin were higher than these of the control group (P<0.05); the difference had statistical meaning in the results of water swallowing test (P<0.05), and the observation group was obviously higher than the control group in the proportion of grade Ⅰ to Ⅲ (P<0.05). ConclusionLow frequency pulse acupoint electrical stimulation and action observation therapy based on mirror neuron system theory is conducive to repairing the damaged nerves in patients, lifting laryngopharyngeal complex mobility, relieving the corresponding clinical symptoms and improving nutritional state of the body.
