摘要:
目的探讨五味消毒饮合当归芍药散治疗下肢骨折患者术后炎症反应的疗效及对术后疼痛、血液流变学的影响。 方法将下肢骨折患者80例采用随机数字表法分为对照组与观察组各40例。对照组口服利伐沙班联合气压，同时应用踝泵训练预防下肢深静脉血栓，观察组在对照组治疗的基础上口服五味消毒饮合当归芍药散。观察两组患者术后肿胀、疼痛、血流流变学等指标。 结果观察组有效率为95.0%（38/40），高于对照组的80.0%（32/40）（P＜0.05）。治疗后观察组肿胀程度评分低于对照组（P＜0.05）。治疗后两组患者血清肿瘤坏死因子α（tumor necrosis factor α，TNF-α）、白细胞介素6（interleukin-6，IL-6）、超敏C反应蛋白（high-sensitivity C-reactive protein，hs-CRP）含量低于同组治疗前，且观察组低于对照组（P＜0.05）。治疗后观察组患者疼痛视觉模拟评分（visual analogue scale，VAS）低于对照组，差异有统计学意义（P＜0.05）。两组治疗后血浆黏度、血液高切黏度、血液低切黏度均低于同组治疗前，且观察组低于对照组（P＜0.05）。 结论五味消毒饮联合当归芍药散能有效减轻下肢骨折术后患者炎症反应，减轻患者术后肿胀与疼痛程度，有助于预防术后感染与深静脉血栓。
Abstract:
ObjectiveTo explore clinical effects of Wuwei Xiaodu drink and Danggui Shaoyao powder in treating postoperative inflammatory reaction in the patients with lower limb fracture and its influence on postoperative pain and hemorheology. MethodsEighty patients were allocated to the control group and the observation group using random number table method, 40 cases each group. The control group took rivaroxaban tablets and pressure, meanwhile, they accepted ankle pump training to prevent deep venous thrombosis of lower extremity, and the observation group were treated by Wuwei Xiaodu drink and Danggui Shaoyao powder. To observe swelling, pain and hemorheological indexes in both groups. ResultsThe effective rate in the observation group was 95.0%(38/40), higher than 80.0%(32/40) of the control group (P<0.05). After treating, the observation group was lower than the control group in the scores of swelling degrees (P<0.05). After treating, the contents of TNF-α, IL-6 and hs-CRP were lower than these before treating in the same group, and the observation group was lower than the control group (P<0.05). After treating, the observation group was lower than the control group in VAS scores (P<0.05). After treating, plasma viscosity, high-shear viscosity and low-shear viscosity were lower than these before treating, and the observation group was lower than the control group (P<0.05). ConclusionWuwei Xiaodu drink and Danggui Shaoyao powder could effectively relieve postoperative inflammatory reaction in the patients with lower limb fracture, improve postoperative swelling and pain degrees, and it is helpful to prevent postoperative infection and DVT.
