摘要:
目的：探讨右归饮联合人工全髋置换术治疗激素性股骨头坏死的临床价值。方法：选择80例患者，分为2组，每组40例，观察组患者在入院后立即服用右归饮，择期实施人工髋关节置换术，对照组则实施常规术前准备后行人工髋关节置换术。对所有患者随访1年，并检测患者术后血脂水平，比较术后骨质疏松、空骨陷窝及骨小梁密度降低发生率及并发症。结果：观察组术后发生骨质疏松、空骨陷窝及骨小梁密度降低的比率均显著低于对照组（P＜0.05），总胆固醇、甘油三酯和低密度脂蛋白水平低于对照组（P＜0.05）；观察组发生假体周围骨折、假体松动者少于对照组（P＜0.05），住院期间发生下肢血栓栓塞和术后感染者少于对照组（P＜0.05）。结论：激素性股骨头坏死实施人工髋关节置换术前及术后使用右归饮能有效调节患者血脂水平，促进术后恢复，减少术后并发症。
Abstract:
Objective: To explore clinical value of YouGui drink and total hip replacement (THR) in treating steroid-induced femoral head necrosis. Methods: Eighty patients were allocated to two groups, 40 cases each group, the patients in the observation group started to take YouGui drink instantly when they were admitted into hospital, and they accepted THR at appropriate time, the control group were operated with THR after preoperative preparation was carried out. All the patients have been followed for one year, the level of blood lipid was detected after the surgery, the complications, the incidences of osteoporosis, lacuna and density decrease of bone trabecula of the patients were compared after the surgery. Results: The observation group was lower than the control group in the incidences of osteoporosis, lacuna and density decrease prominently (P＜0.05), lower than the control group in the levels of TC, TG and LDL (P＜0.05); the observation group was less than the control group in periprosthetic fractures and prosthesis fractures (P＜0.05), the incidences of lower extremities thrombembolia and postoperative infection in the hospitalization period were less than these of the control group (P＜0.05). Conclusion: The patients with steroid-induced femoral head necrosis who received THR taking YouGui drink before and after the surgery could effectively regulate the level of blood lipid of the patients, promote postoperative recovery and reduce postoperative complications.
