摘要:
目的研究蛭蛇通络胶囊对脑梗死后肢体瘫痪患者的疗效及大脑中动脉血流动力学的影响。 方法选择180例脑梗死后肢体瘫痪患者，随机分为观察组和对照组，每组90例。对照组采用常规内科保守疗法，观察组在对照组基础上口服蛭蛇通络胶囊。比较两组上下肢肌力评定情况，临床疗效，肢体运动功能（Fugel-Meyer，FMA）、日常生活活动能力（activities of daily living，ADL）、蒙特利尔认知评估量表（Montreal cognitive assessment Scale，MoCA）及美国国立卫生研究院卒中量表（National institute of health stroke scale，NIHSS）评分，大脑中动脉血流动力学相关指标。 结果治疗后，观察组上下肢肌力为4～5级的人数明显多于对照组（P＜0.05）。观察组总有效率［88.89%（80/90）］明显高于对照组［65.56%（59/90）］。观察组FMA、ADL、MoCA评分明显高于对照组，NIHSS评分明显低于对照组（P＜0.05），且观察组改善程度优于对照组（P＜0.05）。大脑中动脉血流的平均流速、峰流速均明显高于对照组（P＜0.05），大脑中动脉血流的流速差值、峰流速差值均明显低于对照组（P＜0.05）。 结论蛭蛇通络胶囊可改善脑梗死后肢体瘫痪患者的症状及大脑中动脉的血流动力学水平，临床疗效较好。
Abstract:
ObjectiveTo investigate clinical effects of Zhishe collateral-dredging capsules in the treatment of limb paralysis patients after cerebral infarction and its influence on hemodynamics of middle cerebral artery. MethodsAll 180 patients were randomly allocated to the observation group and the control group, 90 cases in each group. The control group adopted conventional conservative therapy of internal medicine, and the observa-tion group was administered with Zhishe collateral-dredging capsules orally based on the therapy the control group accepted. To compare muscle strength evaluation of upper and lower limbs, clinical effects, FMA, ADL, MoCA and NIHSS scores, as well as hemodynamic indexes of middle cerebral artery between both groups. ResultsAfter treating, the number of the people with muscle strength of upper and lower limbs of grade 4-5 in the observation group was notably more than that of the control group (P<0.05); total effective rate of the observation group was 88.89% (80/90), notably higher than 65.56% (59/90) of the control group; FMA, ADL and MoCA scores were higher than these of the control group remarkably, NIHSS scores lower than these of the control group (P<0.05), and the improvements of the observation group were better than these of the control group (P<0.05); mean flow velocity and peak flow velocity of middle cerebral artery higher than these of the control group notably (P<0.05), velocity difference and peak velocity difference of middle cerebral artery blood flow lower than these of the control group apparently (P<0.05). ConclusionZhishe collateral-dredging capsules could improve the symptoms of limb paralysis patients after cerebral infarction, hemodynamic level of middle cerebral artery, and it shows better clinical effects.
