摘要:
目的观察复方斑蝥胶囊辅助治疗对胃癌患者临床疗效及外周血单个核细胞（peripheral blood mononuclear cells，PBMCs）中的核苷酸结合寡聚化结构域样受体蛋白3（nucleotide-binding oligomerization domain-like receptor protein 3，NLRP3）炎性小体表达的影响。 方法将胃癌患者110例按随机数字表法分为对照组和观察组各55例。对照组予FOLFOX4方案化疗，观察组在对照组基础上加用复方斑蝥胶囊辅助治疗。观察两组患者治疗前后PBMCs中NLRP3、半胱氨酸蛋白酶1（Caspase-1）的mRNA及蛋白表达、炎性细胞因子［白细胞介素1β（interleukin-1β，IL-1β）、白细胞介素18（interleukin-18，IL-18）］水平、临床疗效及不良反应发生情况。 结果两组患者治疗后外周血PBMCs中的NLRP3、Caspase-1 mRNA及蛋白表达水平均低于治疗前（P＜0.05），观察组低于对照组（P＜0.05）。与治疗前比较，两组患者治疗后外周血IL-1β和IL-18水平均较治疗前降低（P＜0.05），观察组低于对照组（P＜0.05）。总有效率观察组为74.55%，高于对照组的61.82%（P＜0.05）。观察组治疗后Karnofsky评分改善和稳定的患者比例高于对照组，但差异无统计学意义（P＞0.05）。两组患者不良反应发生率比较差异无统计学意义（P＞0.05）。 结论复方斑蝥胶囊辅助治疗可有效降低胃癌患者NLRP3炎性小体的表达及炎症反应，提高临床疗效。
Abstract:
ObjectiveTo observe clinical effects of compound Banmao capsules adjuvant therapy in the treatment of gastric cancer and its influence on the expressions of NLRP3 in peripheral blood mononuclear cells (PBMCs). MethodsAll 110 patients were separated to the control group and the observation group according to random number table method. The control group accepted FOLFOX4 regimen chemotherapy, and the observation group received compound Banmao capsules adjuvant therapy based on chemotherapy. To observe the expressions of NLRP3 and Caspase-1 mRNA and protein in PBMCs, the levels of inflammatory cytokines [IL-1β, IL-18], clinical effects and the incidence of adverse reaction before and after treatment. ResultsThe expressions of NLRP3 and Caspase-1 mRNA and protein in PBMCs of both groups after the treatment were lower than these before treatment (P<0.05), the observation group was lower than the control group (P<0.05). Compared with before treatment, the levels of IL-1β and IL-18 in peripheral blood reduced after treatment in both groups compared with these before treatment (P<0.05), the observation group was lower than the control group (P<0.05). Total effective rate of the observation group was 74.55%, higher than 61.82% of the control group (P<0.05). After the treatment, the proportion of the patients with Karnofsky improvement and stabilization in the observation group was higher than that of the control group, while the difference had no statistical meaning (P>0.05). The difference had no statistical meaning in the incidence of adverse reaction between both groups (P>0.05). ConclusionCompound Banmao capsules adjuvant therapy could effectively lower the expressions of NLRP3 and inflammatory reaction in patients with gastric cancer, and raise clinical effects.
