摘要:
目的探究温通针法联合补益肝肾方对急性加重期膝关节骨性关节炎（knee osteoarthritis，KOA）患者骨代谢指标、炎症因子水平的影响。 方法将急性加重期KOA患者100例按完全随机法分为温通针组、联合组各50例，温通针组患者给予温通针法治疗，联合组患者给予温通针法联合补益肝肾方治疗，观察两组患者中医证候评分、视觉模拟量表（visual analogue scale，VAS）评分、Lysholm指数、膝关节活动度、骨代谢指标［骨软骨寡聚基质蛋白（cartilage oligomeric matrix protein，COMP）、骨保护素（osteoprotegerin，OPG）、骨钙素（osteocalcin，OC）、核转录因子κB受体活化因子配体（receptor activator for NF-kappaB ligand，RANKL）］、炎症因子［基质金属蛋白酶３（matrix metalloproteinase-3，MMP-3）、肿瘤坏死因子α（tumor necrosis factor-α，TNF-α）、白细胞介素1β（interleukin-1β，IL-1β）、环氧合酶2（cyclooxygenase，COX-2）］水平及临床疗效。 结果治疗后，联合组中医证候评分、VAS评分及COMP、RANKL水平与MMP-3、TNF-α、IL-1β、COX-2水平均低于治疗前及温通针组治疗后（P＜0.05），Lysholm、膝关节活动度及OPG、OC水平均高于治疗前及温通针组治疗后（P＜0.05）。联合组总有效率为98.00%（49/50），高于温通针组的84.00%（42/50）（P＜0.05）。 结论温通针法联合补益肝肾方可有效改善急性加重期KOA患者骨代谢指标，降低VAS评分及炎症因子水平，提升Lysholm、膝关节活动度，效果优于单用温通针法。
Abstract:
ObjectiveTo investigate the influence of Wentong acupuncture and tonifying liver and kidney formula on bone metabolism indexes and inflammatory factor levels in patients with acute exacerbation of knee osteoarthritis (KOA). MethodsOne hundred patients with acute exacerbation of KOA were divided into 50 cases each in Wentong acupuncture group and the combination group according to the complete randomization method. Patients in the Wentong acupuncture group were given Wentong acupuncture treatment, and patients in the combination group accepted Wentong acupuncture and tonifying liver and kidney formula. To observe the TCM syndrome scores, visual analogue scale (VAS) scores, Lysholm index, knee range of motion (ROM), bone metabolism indexes including COMP, OPG, OC and RANKL, inflammatory factors such as matrix metalloproteinase 3 (MMP3), tumor necrosis factor α (TNF-α), interleukin-1β (IL-1β), cyclooxygenase 2 (COX-2), and clinical effects of the patients in both groups. ResultsAfter treatment, the TCM syndrome score, VAS scores and the levels of COMP, RANKL, and the levels of MMP-3, TNF-α, IL-1β, COX-2 of the combined group were lower than these before the treatment and the Wentong acupuncture group after the treatment (P<0.05), and Lysholm, knee ROM, and the levels of OPG, OC were higher than these before the treatment and the Wentong acupuncture group after the treatment (P<0.05). The total effective rate of the combination group was 98.00% (49/50), higher than 84.00% (42/50) of the Wentong acupuncture group (P<0.05). ConclusionThe combination of Wentong acupuncture with tonifying liver and kidney formula can effectively improve the levels of bone metabolism indexes, reduce VAS scores and inflammatory factor, and enhance Lysholm and knee ROM in patients with acute exacerbation of KOA, and the effect is better than that of Wentong acupuncture method alone.
