摘要:
目的探讨温针灸治疗肩周炎的选穴规律。 方法搜集中国知网（China national knowledge infrastructure）、万方数据库（Wanfang data）、中国生物医学文献数据库（sinomed）、维普（Vip data）、PubMed自建库时至2018年12月31日有关温针灸治疗肩周炎的文献，经过筛选，提取数据，并用数据挖掘技术进行分析。 结果纳入108篇文献，涉及处方108条、腧穴39个，选穴总频次为582次。肩髃、肩贞、肩髎是使用频率最高的腧穴，涉及频率最高的经络为手三阳经，占总频率的76.1%。其中近部取穴频率为79.6%，远部取穴频率为21.0%。关联规则分析显示穴位相关性最高的是肩髃-肩贞，支持度为95.37%，置信度为90.29%。聚类分析发现5个有效类群，突出的肩贞-肩前-肩髃穴位群集，与关联规则分析及其他结果高度契合。 结论温针灸治疗肩周炎应重视对手三阳经腧穴的运用，尤其是重视对近部腧穴的运用，配合辨证循经远部取穴。
Abstract:
ObjectiveTo explore the rules of acupoint selection for warming acupuncture treatment of scapu-lohumeral periarthritis. MethodsThe papers about warming acupuncture in treating scapulohumeral periarthritis were searched from CNKI, Wanfang, sinomed, VIP and PubMed between database building and December 31, 2018, the data was extracted after screening and analyzed using data mining. ResultsA total of 108 papers involving 108 prescriptions and 39 acupoints were included, total frequency of acupoint selection was 582 times. Jianyu (LI15), Jianzhen (SI9) and Jiaoliao (SJ14) are the most frequently used acupoints, the most frequently involved meridians were the three Yang meridians of hand, and they held 76.1% of total frequency. Among them, the frequency of acupoint selection in the proximal part was 79.6%, and the frequency of distal acupoint selection 21.0%. Association rules analysis showed that the acupoints with the highest correlation were Jianyu and Jianzhen, support was 95.37% and confidence level was 90.29%. Cluster analysis found five effective groups, the prominent Jianzhen-Jianqian-Jianyu acupoint cluster, which was highly consistent with the association rule analysis and other results. ConclusionWarming acupuncture in treating scapulohumeral periarthritis should pay attention to the application of acupoints of the three yang meridians of hand, especially the application of acupoints selectioin in the proximal part, combined with distal acupoints selection according to sydrome differentiation and meridians.
