摘要:
通过分析相关文献，从“肠-胰岛”轴理论入手，探讨中医健脾益气法治疗脾胃虚弱型糖尿病胃轻瘫（diabetic gastroparesis，DGP）的潜在作用机制。研究认为：糖尿病胃轻瘫以脾虚为本，病位责之脾胃，其发生发展与胃肠激素［生长激素释放肽（ghrelin）、胆囊收缩素（cholecystokinin，CCK）、酪酪肽（peptide YY，PYY）、葡萄糖依赖性促胰岛素多肽（glucose-dependent insulinotropic polypeptide，GIP）、胰高血糖素样肽-1（glucagon-like peptide-1，GLP-1）］分泌紊乱密切相关，契合“肠-胰岛”轴调控胃排空功能的核心病理环节。健脾益气法或可通过下调Ghrelin表达水平，上调CCK、PYY、GIP、GLP-1等胃肠激素含量，进而调节胃排空节律、改善胃肠动力障碍。基于“肠-胰岛”轴激素调控网络，可从“胃肠激素-胃排空”相关性角度，阐释健脾益气法干预脾胃虚弱型糖尿病胃轻瘫的作用机制，为临床从脾论治糖尿病胃轻瘫提供新的研究方向与生物学理论依据。
Abstract:
By analyzing relevant literature and starting from the "gut-islet" axis theory, this paper explores the potential mechanism of action of the spleen-strengthening and Qi-replenishing method of traditional Chinese medicine in treating diabetic gastroparesis (DGP) of the spleen and stomach deficiency pattern. The study suggests that DGP is fundamentally characterized by spleen deficiency, with the pathogenesis rooted in the spleen and stomach. Its onset and progression are closely associated with dysregulation of gastrointestinal hormones, such as ghrelin, cholecystokinin (CCK), peptide YY (PYY), glucose-dependent insulinotropic polypeptide (GIP), and glucagon-like peptide-1 (GLP-1), which aligns with the core pathological mechanism of gastric emptying regulation via the "gut-islet" axis. The therapy of invigorating spleen and benefiting Qi may modulate gastric emptying rhythm and improve gastrointestinal motility disorders by downregulating ghrelin expression and upregulating the levels of CCK, PYY, GIP, and GLP-1. Based on the hormone regulatory network of the "gut-islet" axis, the mechanism of the therapy of invigorating spleen and benefiting Qi in treating DGP of the spleen and stomach deficiency pattern can be elucidated from the perspective of the "gastrointestinal hormone-gastric emptying" correlation. This provides a new research direction and biological theoretical basis for the clinical treatment of DGP from the spleen.
