摘要:
目的观察健脾益肾汤对慢性肾脏病（chronic kidney disease，CKD）4期患者肾功能及肠道菌群的影响，探讨其延缓肾功能进展的机制与肠道菌群的相关性。 方法将60例CKD 4期患者分为治疗组和对照组各30例。对照组采用基础治疗，治疗组在对照组的基础上加用健脾益肾汤，观察治疗前后两组患者主要症状积分，血清尿素氮（blood urea nitrogen，BUN）、肌酐（serum creatinine，SCr）水平及肠道菌群变化情况。 结果治疗后治疗组临床疗效优于对照组，症状改善优于治疗前及对照组治疗后（P＜0.05）；治疗后治疗组血清SCr、BUN水平低于治疗前及对照组治疗后（P＜0.05）；治疗组肠道菌群总厌氧菌、乳酸杆菌、双歧杆菌菌落数较对照组及治疗前增加（P＜0.05），大肠杆菌菌落数较治疗前及对照组治疗后减少（P＜0.01）。 结论健脾益肾汤能够通过调节肠道菌群起到改善CKD 4期患者临床症状，保护肾功能，延缓肾功能进展的作用。
Abstract:
ObjectiveTo study the effects of invigorating-spleen benefiting-kidney decoction on renal function and intestinal flora of patients with stage Ⅳ chronic kidney disease (CKD), and explore the connection between the mechanism of delaying the progression of renal function with intestinal flora. MethodsSixty CKD Patients at stage Ⅳ were divided into the treatment gorup and the control group with 30 cases in each group.The control group adopted basic therapy, and the treatment group took invigorating-spleen benefiting-kidney decoction on the foundation of basic therapy，to observe main symptom scores, BUU, SCr and the changes of intestinal flora in two groups before and after the treatment. ResultsClinical effects of the treatment group were better than these of the control group after the treatment, and the improvements of symptoms of the treatment group were better than these of the control group after the treatment and these before the treatment (P<0.05); the levels of SCr and BUN of the treatment group were lower than these of the control group after the treatment and these before the treatment (P<0.05); intestinal flora total anaerobes, lactobacilli, bifidobacteria colony counts of the treatment group increased than these of the control group and these before the treatment (P<0.05)，E.coli counts reduced than these of the control group and these before the treatment (P<0.01). ConclusionInvigorating-spleen benefiting-kidney decoction could improve clinical symptoms, protect renal function and delay the renal function progression by regulating intestinal flora of patients with stage Ⅳ CKD.
