摘要:
目的探讨消肿定痛合剂联合交联玻璃酸钠治疗气滞血瘀型膝骨关节炎的临床疗效。 方法将120例气滞血瘀型膝关节骨性关节炎患者按照随机数字表法分为3组：A组采用消肿定痛合剂治疗，B组采用交联玻璃酸钠治疗，C组采用消肿定痛合剂+交联玻璃酸钠治疗，每组40例。比较3组治疗4周后（visual analogue scale，VAS）评分、骨关节炎评分（the Western Ontario and McMaster universities，WOMAC评分）和Lequesne评分，并测定血清白细胞介素1β（Interleukin-1β，IL-1β）、肿瘤坏死因子α（tumor necrosis factor-α，TNF-α）、基质金属蛋白酶1（matrix metalloproteinase 1，MMP-1）、超氧化物歧化酶（superoxide dismutase，SOD）水平。 结果VAS评分、WOMAC评分、Lequesne评分及血清IL-1β、TNF-α、MMP-1水平治疗前3组比较，差异无统计学意义（P＞0.05）；治疗后，C组与A、B组比较，差异均有统计学意义（P＜0.05）。 结论消肿定痛合剂联合交联玻璃酸钠治疗气滞血瘀型膝骨关节炎能明显减轻患者膝关节疼痛症状，显著改善膝关节功能，其作用机制可能与减轻炎症反应及过氧化反应有关。
Abstract:
ObjectiveTo explore clinical effects of Xiaozhong Dingtong mixture and crosslinked sodium hyaluronate in the treatment of Qi stagnation and blood stasis pattern knee osteoarthritis (KOA). MethodsAll 120 patients were divided into three groups in light of random number table method, A group was treated with Xiaozhong Dingtong mixture, B group with crosslinked sodium hyaluronate, and C group with Xiaozhong Dingtong mixture and crosslinked sodium hyaluronate, 40 cases in each group, to compare VAS scores, WOMAC scores and Lequesne scores in three groups after four weeks of treatment, and detect the levels of IL-1β, TNF-α, MMP-1 and SOD in the serum. ResultsThe difference had no statistical meaning in VAS scores, WOMAC scores and Lequesne scores, the levels of IL-1β, TNF-α and MMP-1 between three groups before treating (P>0.05); after the treatment, the difference was statistically significant when C group was compared with A group and B group after treatment (P<0.05). ConclusionXiaozhong Dingtong mixture and crosslinked sodium hyaluronate in the treatment of Qi stagnation and blood stasis pattern KOA could notably alleviate pain and significantly improve knee joint function, and its mechanism might be related to reducing inflammation and peroxidation.
