摘要:
目的：收集慢性心力衰竭临床试验报告的替代指标，分析替代指标在选取和使用中存在的问题。方法：检索中国知识基础设施工程(CNKI)中文全文期刊数据库，PubMed等数据库中有关慢性心力衰竭的临床研究文献，依据纳入、排除标准进行筛选，填写中、英文文献调查表，提取文献信息；借助频数分布等方法，分别从研究概况、研究年份、使用频率、组合规律等几个方面统计替代指标的选择和使用情况。结果：纳入文献127篇，收集慢性心力衰竭临床替代指标42种；最常使用的替代指标依次为心功能分级、左室射血分数、N端脑钠肽前体（N-terminal pro-brain natriuretic peptide，NT-proBNP）、6分钟步行试验、生活质量(Quality of Life，QOL)，出现频率最高的替代指标组合为“心功能分级+LVEF”。结论：目前中医临床疗效评价中替代指标的选取不统一、不规范。
Abstract:
Objective: To collect the alternative indicators for clinical trials of chronic heart failure (CHF) and analyze the problems existed in screening and using the alternative indicators. Methods: Literature on clinical research of CHF were searched in CNKI, PubMed and others, screened according to inclusion and exclusion criteria, Chinese and English literature questionnaire were filled to extract literature information; the selection and the use of alternative indicators were counted from research overview, the year, frequency of use, combination laws and other via frequency distribution and other methods. Results: All 127 papers were included, 42 kinds of clinical alternative indicators of CHF were collected; the common alternative indexes were cardiac function classification, left ventricular ejection fraction (LVEF), NT-proBNP, six minutes walking distance and QOL, the combinations of the alternative indicators in the highest frequency were "cardiac function classification+LVEF". Conclusion: The selection of the alternative indicators in the evaluation of TCM clinical effects are not uniform and standard.
