摘要:
目的：系统评价中西医结合治疗慢性肾小球肾炎的临床疗效。方法：计算机检索万方和中国知网中关于中西医结合治疗慢性肾小球肾炎随机对照试验(RCT)。检索时限由建库至2012年12月。提取的相关数据采用RevMan5.0处理。结果：共纳入17个RCT。中西医结合治疗较单纯西药治疗总体疗效好[OR=3.88,95%CI(2.83,5.34)]；治疗组血肌酐降低较对照组明显[MD=-13.88,95%CI(-20.51,-7.24)]，治疗组24h尿蛋白定量降低较对照组明显[MD=-0.43，95%CI(-0.60，-0.26)]。结论：采用中西医结合治疗慢性肾小球肾炎的方法，较单纯采用西医常规治疗方法可更好缓解病情、改善症状，降低24小时尿蛋白量和血肌酐、保护肾脏功能。但鉴于纳入研究少，样本量少，且质量不高，研究评价方法特异性较大，并且存在评价时间的不同，故仍需要开展大样本多中心、方法科学和规范的高质量RCT，以求进一步验证中西医结合对慢性肾炎治疗的临床疗效。
Abstract:
Objective: To assess curative effects of integrative medicine in treating chronic glomerular nephritis(CGN) systematically. Methods: The articles on randomized controlled trial (RCT) of treating CGN by integrative medicine were searched in WangFang and CNKI. Time limit: from creating the database to December, 2012. Extracted related data was dealt with RevMan5.0. Results: Altogether 17 pieces of RCTs were included. Total effects of integrative medicine group were superior to these of western medicine group[OR=3.88, 95%CI (2.83, 5.34)]; serum creatinine (Scr) in the treatment group was decreased notably than that of the control group [MD=-13.88, 95%CI (-20.51, -7.24)], quantitative urinary protein of the treatment group was lower than that of the control group remarkably [MD=-0.43, 95%CI(-0.60, -0.26)]. Conclusion: Treating CGN by integrative medicine could relieve the condition, improve the symptoms, decrease the amount of 24 hours urinary protein and Scr, and protect renal function than the effects of western medicine. Normal high quality RCT with large-sample, multi-centered and scientific method needs to be launched, in order to testify clinical effects of integrative medicine in treating CGN, since the include studies are fewer, the samples are less, the quality is not high, the specificity of individual assessment is large and the evaluation time is different.
