摘要:
目的观察加味养阴通脉饮辅助治疗慢性牙周炎（chronic periodontitis，CP）的临床疗效。 方法将96例CP患者随机分为对照组和观察组，各48例。对照组予龈上洁治、龈下刮治、控制菌斑、根面平整、调整咬合、冲洗牙周袋等基础治疗，并口服甲硝唑片；治疗组在对照组的基础上给予加味养阴通脉饮治疗，两组均治疗4周。比较两组中医证候评分、牙周指数、免疫炎症因子［白细胞介素1β（Interleukin-1 beta，IL-1β）、肿瘤坏死因子α（tumor necrosis factor-α，TNF-α）、C反应蛋白（C-reactive protein，CRP）］水平及临床疗效。 结果治疗后两组牙龈红肿、牙龈脓血性分泌物渗出、疼痛、口干渴、口臭各项中医证候积分均降低（P＜0.05），且观察组低于对照组（P＜0.05）。治疗后两组牙周指数、免疫炎性因子（IL-1β、TNF-α、CRP等）水平均降低（P＜0.05），且观察组低于对照组（P＜0.05）。观察组总有效率［89.58%（43/48）］高于对照组［77.08%（37/48）］（P＜0.05）。 结论加味养阴通脉饮辅助治疗CP有助于改善患者体质，减轻牙周症状，降低免疫炎性指标。
Abstract:
ObjectiveTo observe clinical effects of modified Yin-nourishing pulse-unblocking drink as adjunctive therapy in the treatment of chronic periodontitis (CP). MethodsA total of 96 CP patients were randomized into the control group and the observation group with 48 cases in each group. The control group accepted basic therapy including gingival scaling, subgingival curettage, plaque control, root surface leveling, occlusal adjustment, and irrigation of periodontal pockets, and oral administration of metronidazole tablets; the treatment group was treated with modified Yin-nourishing pulse-unblocking drink on the foundation of the therapy of the control group, and both groups were treated for four weeks. To compare TCM syndrome integrals, perio-dontal index, the levels of immune inflammatory factors including IL-1β, TNF-α and CRP and clinical effects between the two groups. ResultsAfter the treatment, the scores of TCM syndromes including gingival swelling, gingival purulent exudate, pain, dry mouth, bromopnea were lowered (P<0.05), and the observation group was lower than the control group (P<0.05). After the treatment, periodontal index and the levels of immune inflam-matory factors such as IL-1β, TNF-α and CRP were reduced in the two groups (P<0.05), and the treatment group was lower than the control group (P<0.05). Total effective rate of the treatment group was [89.58%(43/48)], higher than [77.08%(37/48)] of the control group (P<0.05). ConclusionModified Yin-nourishing pulse-unblocking drink as adjunctive therapy is conducive to the improvement of the patients' constitutions, the relief from periodontal symptoms and the reduction of immune inflammatory factors.
