摘要:
目的观察壮药白花香莲解毒颗粒治疗HBeAg阳性慢性乙型肝炎的临床疗效及对血浆外泌体miR-122表达的影响。 方法将120例HBeAg阳性的慢性乙型肝炎患者随机分为两组，对照组60例给予恩替卡韦治疗，观察组60例给予恩替卡韦联合白花香莲解毒颗粒治疗，两组均治疗48周，同时选取健康体检者30例作为健康组。观察治疗前后两组临床疗效、乙肝两对半定量、血浆外泌体miR-122、乙肝病毒脱氧核糖核酸（HBV DNA）的水平、肝功能及不良反应事件发生情况。 结果治疗48周，观察组总有效率［90%（54/60）］明显高于对照组［71.67%（43/60）］（P＜0.05）。治疗12、24、48周时，两组HBeAg滴度、血清丙氨酸氨基转移酶水平逐渐下降，且观察组显著低于对照组（P＜0.05）；观察组血浆中miR-122相对表达量高于治疗前（P＜0.05），对照组血浆中miR-122相对表达量与治疗前比较，差异无统计学意义（P＞0.05）。治疗12、24、48周时，两组血清HBV-DNA水平逐渐下降（P＜0.05）；观察组治疗12、24周时HBV-DNA水平低于对照组（P＜0.05），但治疗48周后，两组HBV-DNA水平比较，差异无统计学意义（P＞0.05）。 结论白花香莲解毒颗粒能够抑制HBV-DNA的复制及HBeAg分泌，改善肝功能，其疗效机制可能与调节血浆外泌体miR-122的表达有关。
Abstract:
ObjectiveTo observe clinical effects of Zhuang medicine Baihua Xianglian detoxification granules in the treatment of positive HBeAg chronic hepatitis B and its influence on plasma exosomal miR-122 expressions. MethodsA total of 120 patients were randomized into two groups, 60 cases of the control group were treated with entecavir, and 60 ones of the observation group with entecavir and Baihua Xianglian detoxification granule, both groups were treated for 48 weeks, meanwhile, 30 cases having physical examination were chosen as the healthy group. To observe clinical effects of both groups, quantitative examination of HBV infection serum markers (HBsAg, HBsAb, HBeAg, HBeAb and HBcAb), plasma exosomal miR-122, the levels of HBV DNA, liver function and the incidences of adverse reaction before and after the treatment. ResultsAfter treating for 48 weeks, total effective rate of the observation group was [90%(54/60)], obviously higher than [71.67%(43/60)] of the control group (P<0.05). After the treatment of 12, 24 and 48 weeks, HBeAg titer and serum alanine aminotrans-ferase decreased gradually in both groups, the observation group was lower than the control group significantly (P<0.05); the relative expression of plasma miR-122 in the observation group was higher than that before the treatment (P<0.05), the difference had no statistical meaning when the relative expression of plasma miR-122 of the control group was compared with that before the treatment (P>0.05). The levels of HBV-DNA reduced gradually in both groups when they were treated for 12, 24 and 48 weeks (P<0.05); the levels of HBV-DNA of the observation group were lower than these of the control group when they were treated for 12 and 24 weeks (P<0.05), while the difference had no statistical meaning in the levels of HBV-DNA after treating for 48 weeks. ConclusionZhuang medicine Baihua Xianglian detoxification granules could inhibit the replication of HBV-DNA and the secretion of HBeAg, improve liver function, and its clinical effects might be related to regulating the expressions of plasma exosomal miR-122.
