摘要:
目的：观察参苓白术散联合氨茶碱片治疗慢性阻塞性肺疾病的临床疗效。方法：将72例慢性阻塞性肺疾病患者随机分为两组，每组36例。给予两组吸氧、平喘、化痰、止咳和抗感染治疗。对照组口服氨茶碱片，0.1 g/次，每日3次。观察组在对照组基础上服用参苓白术散，每日1剂，水煎分服。两组均治疗3个月。比较两组患者肺功能、营养情况、住院时间及病死率。结果：总有效率观察组为91.67%（33/36），对照组为77.78%（28/36），两组比较差异有统计学意义(P＜0.05)。治疗后两组体质指数及白蛋白水平均明显升高(P＜0.05)，且观察组升高更为明显(P＜0.05)。观察组的住院时间及病死率均明显低于对照组(P＜0.05)。治疗后，两组每分钟最大通气量(maximum ventilatory volume，MVV)、6 min步行距离和用力呼气量占用力肺活量比值(FEV1/FVC)均明显升高(P＜0.05)，且观察组更为明显(P＜0.05)。结论：参苓白术散联合氨茶碱片对于慢性阻塞性肺疾病患者具有满意的治疗效果，可以有效改善患者机体营养、肺功能及运动量。
Abstract:
Objective: To explore clinical effects of Shenling Baizhu powder and aminophylline tablets in the treatment of the patients with chronic obstructive pulmonary disease (COPD). Methods: All 72 patients were randomized into two groups, 36 cases each group. Both groups accepted the therapy including oxygen inhalation, relieving asthma, reducing phlegm, stopping cough and fighting against infection, the control group took aminophylline tablets, 0.1g each time, three times each day. The observation group were given Shenling Baizhu powder simultaneously, one dose each day, water decocted. Both groups were treated for three months. To compare pulmonary function, nutritional conditions, hospitalization time and mortality between both groups. Results: Total effective rate of the observation group was 91.67% (33/36), higher than 77.78% (28/36) of the control group, and the difference had statistical meaning (P＜0.05). After treating, body mass index and albumin level of both groups rose notably (P＜0.05), and the increase of the observation group was more notable (P＜0.05). The observation group was lower than the control group in hospitalization time and mortality notably (P＜0.05). After treating, MVV and six minutes walking distances and FEV1/FVC of both groups rose notably (P＜0.05), the rise of the observation group was more notable (P＜0.05). Conclusion: Shenling Baizhu powder and aminophylline tablets own satisfactory therapeutic effects in the treatment of COPD, it could effectively improve body nutrition, pulmonary function and the amount of exercise.
